Requirements TheLead Care Managerwill ensure that: · Successfully improves the efficiency, productivity and performance of the care management team. · Increases care management team external engagement through partnerships. · The care management team is in compliance with State standards. Education And Experience Required: · Excellent communication skills and ability to form collaborative partnerships across all service settings. · Working knowledge of the provision of health care in a variety of settings. · Knowledge of community resources required. · Preferred experience: 3-5 years of clinical nursing or social work experience, preferable with 3-5 years of community health experience, with the adult population. · Ability to assimilate new information and technologies into daily work. · Competent in Microsoft Office products (Word, Excel, Outlook, PowerPoint) Licenses and Certifications: · Must have a valid driver’s license and reliable transportation. · BLS through the American Heart Association · Infection Control certificate Preferred Qualifications: · Demonstration of leadership and or supervisory experience · RN, current NYS license, or MSW.
