As one of the first 5 Federally Qualified Health Centers (FQHC) established in the nation, its roots are steeped in service to those who face barriers to health and health care, meeting their need for comprehensive medical, dentistry, behavioral health, and community services. Starting in northeast Rochester, NY, Jordan Health has since expanded to become a network of primary care offices and health centers serving residents living in three quadrants ofRochester, NY (http://www.city-data.com/city/Rochester-New-York.html) and in Canandaigua, N.Y.
Jordan Health is now seeking aLead Care Managerwho, under the guidance of the Director of Social Emotional and Mental Health, is primarily responsible for providing oversight of training and guidance to the care management team.

Requirements
TheLead Care Managerwill ensure that:
· Successfully improves the efficiency, productivity and performance of the care management team.
· Increases care management team external engagement through partnerships.
· The care management team is in compliance with State standards.
Education And Experience Required:
· Excellent communication skills and ability to form collaborative partnerships across all service settings.
· Working knowledge of the provision of health care in a variety of settings.
· Knowledge of community resources required.
· Preferred experience: 3-5 years of clinical nursing or social work experience, preferable with 3-5 years of community health experience, with the adult population.
· Ability to assimilate new information and technologies into daily work.
· Competent in Microsoft Office products (Word, Excel, Outlook, PowerPoint)
Licenses and Certifications:
· Must have a valid driver’s license and reliable transportation.
· BLS through the American Heart Association
· Infection Control certificate
Preferred Qualifications:
· Demonstration of leadership and or supervisory experience
· RN, current NYS license, or MSW.
