Job Overview:
The Team Leader supports operational efficiency of the Case Management Department to ensure seamless management of the day-to-day activities of all Case management staff and activities, including but not limited to discharge planning, care coordination activities, social work, Utilization Review, and Observation units (if there is one).

Qualifications:

Required:

Graduate from an accredited school or universityBSN degreeWorking knowledge of utilization review and discharged planning, hospital case management experience requiredProficiency in MCG guideline applicationKnowledge of regulatory requirements, state and federal guidelines and conditions of participation

Preferred:

Case Management certification, or required within 1 year of hire

Certifications and Licenses Required:

Current Registered Nurse License in the state of New JerseyBasic Life Support (BLS) from the American Heart Association

Scheduling Requirements:

Full-TimeDay shift40 hrs. weeklyMonday Friday8a 4:30pSome weekends if necessaryMust be flexible

Essential Functions:
Reduces LOS through the rounding ensuring case manager collaboration with all members of the interprofessional team by addressing anticipated LOS, estimated date of discharge, and barriers to achieving improved hospital throughput. Ensures workflow, policy and procedures are followed. Provides support oversight and leadership to the Case Management team.

Quality & Fiscal Management
Ensures that appropriate resource utilization is maintained.
Demonstrates an understanding of level of care changes and the financial impact.
Participates in performance improvement initiatives
