Lead Case Manager

With the belief\u202fthat they were there when we needed them, so we must be there now that they need us.\u202fVeterans Inc. is the largest provider of supportive services to Veterans and their families in New England. We are seeking to fill the Lead Case Manager position and add valuable members to our team to accomplish our mission!\n

In response to the rising homeless population throughout Massachusetts, we have been awarded a grant under the Housing Outreach-to-Placement Effort Request for Responses (HOPE). As part of this initiative to expand our reach, we are looking for a Lead Case Manager.\n

WHAT YOU'LL BE RESPONSIBLE FOR DAY TO DAY:\n\nPerforms initial intake and assessments of all veterans for acceptance into Veterans Inc. programs.\nDevelops individualized service plans for homelessness prevention and or rapid rehousing based on veterans input, interests, and needs. Coordinates veterans' participation in the agency programs.\nAssists veterans with researching and accessing needed services including VA benefits, employment training\/placement , educational support, counseling, housing, medical services and other public benefits such as health care services, daily living services, personal financial planning services, transportation, fiduciary and payee services, legal services, child care, housing counseling services, and temporary financial assistance.\nTracks and reports case management progress through case notes and conferences with Program Manager and coordinates with all funding sources.\nCompletes record keeping\/documentation responsibilities and compiles data for monthly reporting into the agency database.\nConducts outreach activities to market Veterans Inc. programs and engage new referral sources.\n\n

WHAT YOU MUST HAVE:\n\nBachelor's degree in Human Services, Social Work or other related field required.\nMinimum of two years' of relatable experience in Employment Training, Vocational Rehabilitation, Recruitment, Counseling, Outreach\/and or Services to the Homeless, or related field, is required.\nStrong computer skills, including Microsoft Word, Excel, and Outlook.\nMust be able to complete CPR\/AED and First Aid certification and maintain valid certification through duration of employment.\nFrequent travel is required. Must have a valid driver's license and own personal, reliable transportation.\nMust be able to successfully pass a CORI\/Background records check.\nCompliance with Veterans Inc. COVID-19 Vaccination Policy. Religious and Medical Exemptions are considered.\n\n
