Lead Clinical Pharmacist

Position Summary:

Under the direction of the Director of Pharmacy, the Lead Clinical Pharmacist assumes responsibility for leading pharmacists’ activities across the continuum of Inpatient and Outpatient care.  In collaboration with the Director of Pharmacy, this position will assist in the ongoing review, maintenance, and enhancement of pharmacy operations and patient care activities in the inpatient setting, the outpatient care areas (including but not limited to (hematology/oncology, urgent care, hospital licensed clinics, etc.).  The Lead Clinical Pharmacist will continue to participate in patients’ pharmacotherapy across the continuum of the medication use process including the filling and dispensing of medication orders; ensuring the proper handling, usage and storage of drugs in the Pharmacy, maintaining standards set by the department, and translating policies and procedures into the daily operation of the Pharmacy. Responsible for acting as an information resource to patients, physicians, nurses and other health care professionals regarding medication therapy and drug related information and literature. Ability to plan and organize work schedule of support personnel as required.

Reasonable accommodations may be made to enable individuals with disabilities to perform the primary responsibilities.  Works with Director to assure adequate staffing for effective provision of pharmacy services for all patients, providers, and administrators within the Signature Healthcare group.  Coaching others in regard to new procedures, new equipment, and life-long learning to maintain proficiency with current pharmacy practice as identified by pharmacist standard of care, etc.  Review medication therapies for appropriate dose for patients from neonates to geriatrics. Knowledge of medication selection, dosing, pharmacokinetics, medication reconciliation, identification of drug-drug interactions, nutritional support, and application of this knowledge to specific patients.  Review and assess adverse drug reaction reports related to causality and severity.  Engages with other hospital departments to optimize an interdisciplinary approach to problem solving and optimal patient care.  Participates in department and hospital quality initiatives to enhance patient care, colleague satisfaction and coordinating all department activity allowing department colleagues to work at or near the top of their licensure (ex. establishment of a career ladder for pharmacists and technicians)  Works with Director of Pharmacy and quality department to develop dept metrics addressing key elements of pharmacy practice for both distribution and patient care activities (orders processed, IV preparation and accuracy, Omni cell fill and accuracy, consults completed, interventions documented, kinetic dosing per protocol with documentation in the patient record, etc.).  Assists with coverage for additional activities in the absence of the Director.  Leads all department colleagues (pharmacists and technicians) toward continuous adherence to hospital and department policies and procedures as well as local, state, and national regulatory requirements and coordinates the Department's activities in the areas of patient care (clinical) and distribution functions.  Performs other duties as assigned  It is reasonable to anticipate that the incumbent will have occupational exposure to bloodborne pathogens in the course of his/her regularly scheduled activities as an associate of Brockton Hospital. 

 

BASIC KNOWLEDGE/SKILLS/APTITUDE/EXPERIENCE:

Ability to solve practical problems and deal with a variety of variables in situations where only limited standardization may exist.    Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form.    Must be able to reasonably make appropriate judgment in communications and actions with patients, physicians, other associates, outside agencies, and vendors.  Knowledge of State and Federal regulations governing drug purchasing, storage, and dispensing.   Knowledge of pharmacology, therapeutics, and use of primary literature to expand that knowledge as needed for patient or provider need.   Knowledge of non-sterile and aseptic procedures in accordance with USP 795/797/800, IV medication compatibilities, and appropriate expiration dating for all prepared or pre-packaged products.  Knowledge regarding the specific care of patients with one or more specific clinical conditions (cardiac, infectious disease, hematology/oncology, endocrine, etc.). 

 

Education/Experience/Licenses/Technical/Other:

1.Education:  Bachelor of Science Degree in Pharmacy required. Doctor of Pharmacy or Bachelor of Science with ASHP accredited Pharmacy Residency, or equivalent experience required.  

2. Experience (Type & Length):  Four years’ experience as a Registered Pharmacist in a hospital setting preferred  

3. Certification/Licensure:   Must be a licensed pharmacist in the State of Massachusetts.
