Summary
The Lead DSS will balance an assigned workload and provide mentoring to the DSS staff as necessary to ensure compliance with discharge planning standards. This role will act as a first line resource for the DSS staff and the first point of contact for issues related to daily activities The Lead Discharge Support Specialist is also responsible for performing administrative and operational functions required in the Care Management Department to support regulatory requirements and patient throughput activities to ultimately ensure patients receive the treatment and services they require post-discharge and are informed of their rights and choice options during the process. The Discharge Support Specialist role oversees the administrative operations of the Care Management Department and acts as the administrative liaison between all clinical departments, community resources, and third-party payers/insurance companies in support of the work of the department.
REQUIREMENTS/QUALIFICATIONS:
Associate's Degree preferred Previous clerical experience in a healthcare setting. Experience with office equipment to include Microsoft Suite, and electronic medical record systems required Experience in care coordination, Care Management, Disease Management, hospital/acute care setting, or customer service requiredAdditional Information
