Lead Integrated Care Coordinator Job Details Job Location Adelante Healthcare Center Support Office - Phoenix, AZ Position Type Full Time Education Level HS Diploma from accredited school Job Category Health Care Description POSITION SUMMARYThe Lead Integrated Care Coordinator supervises the Integrated Care Coordinator work force, coordinates, and assists the Integrated Health Managers in standardizing the work operations of the integrated clinical staff. Primary responsibilities include assisting the Integrated Health Services Director and Integrated Health Managers in Dietetics and Behavioral Health Consultations with administrative tasks, supervising the integrated care coordinators, maintaining policies, standards, and programs for the provision of optimal, cost-effective patient care; monitoring performance for safety, efficacy, and evidence-based practice; recruiting, hiring, and orienting staff; interacting with the Integrated Health Managers and organization’s management and professional staff; and maintaining professional competency and skills required for professional practice. This position is essential to Adelante’s team-based primary care medical home model of care that supports integrated, coordinated, patient-centered, comprehensive health care to all. OUR CORE VALUES• Inclusion• Nurture• Service• Purposeful• Integrity• Resilient• Engaged Qualifications ESSENTIAL SKILLS AND EXPERIENCE:• Bachelor’s degree in social work, behavioral health or psychology field is preferred or a minimum of three (3) years of behavioral health experience and five (5) year of direct experience may replace bachelor’s degree• High school diploma or GED from an accredited institution is required• Minimum of two (2) years of experience working with State AHCCCS and Regional Behavioral Health Authority (RBHA) clients preferred• Excellent customer service skills• Excellent computer skills• Knowledge of chronic disease management• Ability to complete PRAPARE assessments• Comprehensive and working knowledge of community resources• Ability to complete a Behavioral Health Assessment with appropriate clinical oversight• Ability to provide effective and respectful quality care and services that are responsive to diverse cultural beliefs and practices• Certification to perform cardiopulmonary Resuscitation for the Health Care Professional (CPR) and AED through courses that follow the guidelines from the American Heart Association and Red Cross• Bilingual English/Spanish strongly preferred• Valid Arizona driver’s license• Valid Level One Fingerprint Clearance Card issued by Arizona Department of Public Safety• Travel to multiple locations may be requestedPOSITION REPONSIBILITIES/ESSENTIAL:• Work in close collaboration with the Integrated Behavioral Health Manager, Integrated Dietitian Manager, and Integrated Health Services Director to support the Integrated Health Services meet standards in patient care and patient flow.• Assist in interviewing, hiring, and facilitating training of new employees to meet department staffing needs.• Serve as a mentor to facilitate professional growth and job satisfaction of employees.• Assist in assessing, designing, implementing, evaluating, and creating/updating resources.• Update Integrated care coordinator communication channels regularly to ensure integrated care coordinators are up to date on departmental and organizational changes.• Assist in managing administrative managerial tasks such as timecard approvals, scheduling adjustments and tracking departmental projects.• Clearly communicate and coordinate relevant patient information amongst the care team and treating clinicians to maintain proper patient care plans.• Refer to appropriate interprofessional team members, resources, or programs as needed.• Assist the Integrated Behavioral Health Manager in the completion of appropriate reports as needed.• May develop relationships with external healthcare organizations and facilities in efforts to find additional resources for enhanced patient care.• Attend trainings, and conferences for continued professional development.• Participate in three (3) patient care days per week.• May provide telehealth education using landline/Jabber soft phone, or Zoom used via desktop, or personal mobile device.
