POSITION SUMMARY
The Lead Integrated Care Coordinator supervises the Integrated Care Coordinator work force, coordinates, and assists the Integrated Health Managers in standardizing the work operations of the integrated clinical staff. Primary responsibilities include assisting the Integrated Health Services Director and Integrated Health Managers in Dietetics and Behavioral Health Consultations with administrative tasks, supervising the integrated care coordinators, maintaining policies, standards, and programs for the provision of optimal, cost-effective patient care; monitoring performance for safety, efficacy, and evidence-based practice; recruiting, hiring, and orienting staff; interacting with the Integrated Health Managers and organization’s management and professional staff; and maintaining professional competency and skills required for professional practice. This position is essential to Adelante’s team-based primary care medical home model of care that supports integrated, coordinated, patient-centered, comprehensive health care to all.
