Lead Medical Clinician, Nurse Practitioner

POSITION SUMMARY
The School-Based Health Center Department (SBHC) collaborates with three school districts to provide comprehensive health services at eight SBHCs with the mission to reduce health inequities by focusing on prevention and youth empowerment. The Lead Medical Clinician will be a valuable member of the NAHC SBHC team by providing high quality medical care and clinical leadership to deliver comprehensive primary, preventative, chronic and urgent care to NAHC’s members. The Led Medical Clinician will work as part of a multi-disciplinary team and will work under the direct supervision of the Director of School-Based Health Care and the Chief Medical Officer or designee.

The Native American Health Center is an accredited institution and adheres to the standards of excellence set forth by the Accreditation Association of Ambulatory Health Care (AAAHC) and the Commission of Dental Accreditation (CODA).

DUTIES AND RESPONSIBILITIES
Clinical Responsibilities
1. Provide primary care to members, including histories, exams, lab tests, prescriptions and referrals within prescribed up to date guidelines and protocols.
2. Perform accurate and timely documentation of histories, subjective findings, physical exam, plans, prescriptions and follow up in Electronic Health Records System within 72 hours of the visit.
3. Provide members information concerning their evaluation, diagnosis, treatment and prognosis. Engage members in participation and decision involving their healthcare.
4. Adhere to medical visit model of NAHC; managing huddles, proactive in teaching MA’s, seeing members on time, communicating with supervisors any barriers to this.
5. Timely coordination of care among internal and external ancillary providers and hospitals to provide high quality integrated care between departments, specialty care and community resources. This includes referring members and co-managing members with Nutritionists, Care Coordinators, Dentist, LCSWs, etc.
6. Function within an interdisciplinary team and participate in interdisciplinary activities such as panel management and case conferences.
7. Report births, deaths, and outbreaks of diseases to proper authorities.
8. Ensure completion of medication refills, provider approval queue, inbox items, medical record approval and member forms within five (5) working days.
9. Be an active educator of medical team members, including Nurse Practitioners, Physician Assistants, Medical Assistants, Nurses, and other integrative services team members.
10. School Based Health Centers: If assigned to school-based clinics the following duties may apply:
a. Participate in school site meetings and work with health center and school staff to develop and facilitate integrated youth services and activities. This includes facilitating group interventions around site-driven topic areas in order to increase access and build capacity (i.e obesity interventions, LGBTQ youth, etc) as needed.
b. Conduct classroom and community presentations on a variety of health topics as part of outreach efforts to expand clinic services.
c. Provide case management and PCP communication for students who have chronic health issues who are outside of our health network.
d. Communicate with parents regarding non-confidential health issues and provide instructions for care.
e. Develop conference abstracts and proposals and present best practices at local, state and national SBHC conferences as requested.
f. Support NAHC 7 Direction clinic with special projects as assigned.
11. Member Care: Demonstrate understanding and apply working knowledge of safety policies and ensuring safe member practices.
12. Quality Improvement: Actively participate in internal quality improvement teams and proactively work with clinical data team and members to drive quality improvement initiatives in accordance with the mission and strategic goals of the
organization, federal and state laws and regulations, and accreditation standards, when assigned.
13. Safety: Responsible for ensuring that all duties, responsibilities and operations are performed with the utmost regard for the safety and health of all personnel involved, including themselves.
14. Safety: Take appropriate corrective actions to address matters pertaining to employee health and safety that have been brought to their attention.
15. Safety: Must maintain compliance with ergonomic safety standards; be mindful of posture and regularly practice ergonomic stretches.
16. HIPAA: Keep all protected health information (PHI) confidential and abide by HIPAA guidelines for the release and disclosure of any PHI. Will report unauthorized use of disclosure of PHI immediately, to Site Director and HIPAA security officer.
17. Other duties as assigned by Supervisor.

Lead Clinician Duties
18. In collaboration with the Director of School-Based Health Care, the Chief Medical Officer or their designee, and SBHC Program Managers, develop, implement and evaluate department goals and objectives in the following areas:.
a. Liaisonship: Ensure alignment between NAHC Medical Department and SBHCs, serving as liaison between sites to facilitate standardization of clinical standards of care, quality improvement practices and compliance tactics. When appropriate, disseminate best practices and key learnings from SBHC sites to advance organizational clinical and QI performance in pediatric care.
b. Quality: Identify, implement and evaluate quality improvement initiatives to advance clinical quality performance to align with agency goals and nationwide best practices in adolescent and family care.
c. Care: Support growth and development of clinical department staff, including Nurse Practitioners, RN’s and medical assistants.
d. Training and Development: Identify and implement trainings for SBHC staff in support of clinical quality and compliance goals.
e. Staff Supervision: Support with clinical staff onboarding and perform annual evaluation for clinical staff incorporating site staff feedback.
f. Community Partnership: Under supervision of NAHC leadership and staff, identify opportunities to create Student and Family Advisory board(s), collect and disseminate care experience data, and use care experience
data to drive improvement in and equity of patient care services.
19. Effectively lead and retain nurse practitioner staff by fostering a team environment and providing opportunities for staff to develop new skills and promoting clinical and operational staff well-being. When appropriate, facilitate clinician leadership development alignment with organizational priorities and needs.
20. Resolve staffing issues and facilitate conflict resolution for staff under direct supervision. Hire, discipline, evaluate and terminate staff in coordination with SBHC Director and the Human Resources department.
21. In collaboration with Director of Clinical Services, Director of School Based Health, and SBHC Clinics Manager, review and analyze provider productivity and make recommendations to improve access and member flow. Assess barriers and train providers on how to improve their productivity.
22. Collaborate with administration in addressing staff and member complaints.
23. Actively participate in peer review and other internal quality improvement efforts and work with members proactively to drive quality improvement initiatives in accordance with the mission and strategic goals of the organization, federal and state laws and regulations, and accreditation standards, when assigned.

Scope of Work
24. Be able to see between 12-18 members per day, and complete accurate documentation within 72 hours of visit into the electronic health record (EHR) system.
25. Be able to complete medical refills, member approval queue, inbox, medical records approval and member forms within five (5) working days.
26. To work as medical team assisting other providers in completing their responsibilities as necessary, e.g., seeing members, processing refills, clearing member approval queue, and provider inbox.
27. Effectively manage a member panel up to the goal set by the organization.
28. Display the highest level of professionalism, integrity and a strong work ethic, serving as a model for all clinic staff.
29. Maintain requirements of licensure, DEA certification and board certification if hired as board certified. Fulfill and maintain requirements for credentialing, privileging internally at NAHC and externally with payers and regulatory bodies.
30. Provide comprehensive, high quality medical services to members, regardless of age, sex, gender identification, income, national origin or language.
31. Consider individuals’ cultural, psychological, social and environmental factors in creating treatment/care plans.
32. Actively participate in meetings as assigned by direct supervisor.
33. Participate in trainings of other medical personnel as requested.
34. Participate in Saturday clinics, after-hour clinics and on-call duties, on a rotating basis, as requested by supervisor.
