servicesJob SummaryThe Lead Medical Navigator works in coordination with interdisciplinary teams to maintain efficient patient flow and quality of care The role assumes staffing and leadership needsJob ResponsibilitiesProcess and transition clients through admissions including completion of all required documentation Meet with clientrsquos family to obtain information related to clientrsquos history as it pertains to admissionCoordinate with all admissions related staff to expedite the clientsrsquo admissions process across Utilization Review Nursing Admission Specialists and other related departmentsObtain urine toxicology screens take client vitals and report abnormal valuestrends to staff nurse Complete safety checks throughout the clients stayOrient clients to the rules and expectations for clients at Rushford Provide basic counseling to help decrease the client anxiety and assure continuity of careManage clientsrsquo discharge by ensuring all property is flow etcPrevious experience in drug alcohol treatment or disease concept of addiction preferredPrevious experience with mental health andor psychiatric population preferredFamiliarity with Alcoholics
