Lead Medication Adherence Specialist

The Lead, Medication Adherence Specialist is responsible for handling outreach to patients by gathering patient information, fulfilling patient needs, documenting interactions and educating the patient as appropriate to help reduce the need for additional contact on same request. The incumbent in this role confers with patients and/or family members to reconcile a comprehensive list of the patient medications and schedules. Researches by contacting caregivers and pharmacies. He/She leads a group of Medication Adherence Specialists in performance management, training and troubleshooting for the Medication Adherence Call Center.

ESSENTIAL JOB DUTIES/RESPONSIBILITIES:

Makes outbound calls to patients and healthcare professionals regarding patient medications, adherence and barrier removal. Interacts with healthcare professionals concerning patient medications. Calls the pharmacy or provider to assist patient with medication refills, medication synchronization or 90-day supply requests.Facilitates and/or coordinates the process whereby prescriptions are sent into the pharmacy for patients, processed appropriately, adjudicated to third party insurance, error resolution, completed, packaged and prepared for dispensing to the patient.Ensures assigned prescription processing tasks are accurate and reviewed by supervising personnel as required before dispensing to patients.Leads a group of Medication Adherence Specialists in performance management, training and troubleshooting.Works to remove barriers and ensure prescription regimens are provided to patients. This may involve analysis, research, and problem-solving to obtain resolution.Follows established medication adherence workflows, including but not limited to: following up with patients requesting enrollment and patients currently enrolled to initiate or continue delivery of medication regimens.Captures and supports medication adherence patient prescription requests/needs.Trains new team members on department process and policy.Completes minimum daily goals for outreach (telephonic, email, mail) to patients and clinicians for Medication Adherence measures.Serves as a department resource to both the Medication Adherence Team and Organizational Leadership.Provides feedback to both team members and management regarding solutions on how to improve the medication adherence patient/client experience.Performs other duties as assigned and modified at manager’s discretion.

Proficient skill in Microsoft Office Suite products including Excel, Word, PowerPoint and Outlook; competent in other systems required for the position

Ability and willingness to travel locally, regionally and/or nationally up to 10% of the time; flexible to work evening, weekends and/or holidays as needed

Spoken and written fluency in English

Ability to make decisions or on complex issues regarding job duties and related tasks

Works under minimal supervision, and analyzes variable factors to determine the best course of action

EDUCATION AND EXPERIENCE CRITERIA:

High school diploma or equivalent GED requiredAA/AS degree in a healthcare related field preferredA minimum of 3 years’ work experience in a medication dispensing/pharmacy setting requiredCertification as a Pharmacy Technician (CPhT) or Medication Assistant (CMA) required
