Due to organizational growth, Liberty Healthcare is hiring for a new full-time Lead Options Counselor position. As a Lead Options Counselor, you will be an active contributor with a statewide Aging and Disability Resource Center system which supports older individuals and people who are living with disabilities. Program objectives include: Functioning as the “go to” resource for people who are seeking information on long term care and other community-based services and supports Providing person-centered planning in a way that supports informed decision making and the ability to secure services which will enable people to live as independently as possible Your day-to-day responsibilities as a Lead Options Counselor will involve: Leading a team of six field-based Options Counselors Ensuring a significant number of Kansans receive high quality options counseling services Data analysis and reporting Participating in quality assurance initiatives Providing guidance and consultation on complex matters Meeting with participants and their families in-person and virtually Assessing participant’s needs, providing information, and answering questions Connecting people with appropriate services and supports Documenting interactions, services, and relevant information in computer systems This is a hybrid position in which you will work in-person in Liberty Healthcare’s office in Overland Park, travel throughout Kansas, and work from your home office.
