This is a Salaried position. Overview Day shift: 7 on 7 off work schedule; 6:00am-4:30pm; 10 hour days. workng every other weekend/holiday This position will be working at OSF St. Francis Medical Center in Peoria, IL POSITION SUMMARY: The Lead Clinical Pharmacist, according to department policies and pharmaceutical care standards, assists in overseeing and directing pharmacy activities on assigned shifts in order to ensure the highest quality patient care. Determines work priorities, assists in staff assignments and helps to ensure adherence to policies, procedures, performance standards and service expectations. Responsible for compounding and dispensing medications and other pharmaceutical supplies according to established procedures and professional pharmacy practice. Reviews prescriptions, obtains clarifications, analyzes drug therapy and regularly consults with medical staff, nursing personnel and others in order to coordinate delivery of high quality pharmaceutical services to patients. Demonstrates the knowledge and skills necessary to provide patient care that is appropriate to the ages of the patients served. PRIMARY JOB RESPONSIBILITIES AND ACCOUNTABILITY: Manages and improves the medication-use process by reviewing prescription orders and ensuring orders are safe, appropriate, cost-effective, and processed timely according to established policies, procedures and protocols Performs duties of clinical pharmacist, including preparation of prescription orders, sterile and non-sterile compounds or oversight of pharmacy personnel performance of the same. Verifies correct labeling, packaging and technique is used in the preparation and dispensing of medication processes. Maintains records pertaining to all phases of medication and supply ordering, storage and distribution as required by OSF HealthCare, governmental and accrediting agency regulations Provides evidence-based, patient-centered medication therapy management according to approved protocols of Pharmacy Services (pharmacokinetics, warfarin dosing, IV to PO, renal dosing, etc.) and recommends/monitors appropriate lab studies needed to determine whether therapeutic objectives are being obtained). Participates in multi-disciplinary patient care rounding as needed and assists other health care providers in identifying and resolving patient care issues associated with drug therapy. Collaborates with and acts a resource for providers, nurses, case management and other members of the patient care team, including patient counseling and education when required. Aids in data collection for the preparation and reporting of analysis metrics, along with proactively analyzing and interpreting the data and making recommendations for the entity to Pharmacy leadership. Prioritizes and distributes work in conjunction with leadership, including work schedules and mentoring, of personnel engaged in production and dispensing of pharmaceutical products. Provides input and assistance in personnel performance appraisals. Assists in ensuring pharmacy compliance with laws, standards, and regulations. Provides input on development, implementation and review of policies and procedures Qualifications REQUIRED QUALIFICATIONS: Licenses/Certifications: PHRM-Pharmacist Experience: 2 years of hospital pharmacy experience PREFERRED QUALIFICATIONS: Education: Doctorate of Pharmacy (PharmD.) PGY1 residency Experience: 2 years of leadership experience Licensure/Certifications: Board Certification through Board of Pharmacy Specialties Other requirements/information: Demonstrated excellence of interpersonal skills in order to effectively mentor and educate assigned personnel, facilitate/organize/direct the work of others and to maintain positive relationships among leadership, ministry/facility staff and internal/external stakeholders Communicates effectively with physicians, patients, and various other OSF HealthCare personnel when explaining medication use, and gathering and exchanging other medication information Analytical skills necessary in order to determine drug appropriateness and evaluate drug interactions Demonstrated skills in independent problem solving, decision‑making abilities, and prioritization/delegation Demonstrated knowledge of the principles of growth and development as well as the physical, emotional, and psychosocial needs of neonates, pediatrics, adults, and geriatrics Excellent computer skills and the ability to process detailed information in an accurate manner Competency in the use of an electronic medical record system.
