POSITION SUMMARY: The Structured Day Program Coordinator is responsible for developing, implementing, and overseeing person-centered services for individuals with Traumatic Brain Injuries (TBI) and other eligible participants under the New York State Medicaid Waiver for TBI and Nursing Home Transition & Diversion (NHTD) programs. This role ensures programming aligns with waiver requirements, individual care plans, and best practices in supporting individuals with cognitive, physical, and emotional challenges. The coordinator will supervise staff, manage program operations, plan and facilitate engaging activities both within the facility and in the community, and ensure a safe, inclusive environment for all participants.
HOURS: Monday-Friday, base of 45 hours weekly with potential for weekend days

Education:
Education: Program staff must be at least 18 years old with a minimum of a High School Diploma or equivalent (i.e. GED); be able to follow written and verbal instructions; and have the ability, skills, training and supervision necessary to meet the waiver participant’s needs that will be addressed through this service to assure the health and welfare of the waiver participantCertified New York State Personal Care Aide CertificationExperience: 2 years prior experience working with individuals with TBI, cognitive impairments, elderly, disabled and/or neurodiverse populations. Familiarity with New York State’s TBI and NHTD Medicaid Waiver programs. Experience in program coordination, community engagement, and structured day services.KNOWLEDGE AND SKILLS:
Strong leadership, crisis intervention, and team management abilities.Effective problem-solving and conflict-resolution skills.Knowledge of TBI-related challenges, including memory loss, impulsivity, executive dysfunction, and emotional regulation.Ability to identify and secure external partnerships and resources to expand program offerings.Proficiency in Microsoft Office, electronic documentation systems, and Medicaid compliance tracking.CUSTOMER SERVICE/INTERPERSONAL SKILLS

Trauma-Informed & Person-Centered Care: Expertise in supporting individuals with TBI, cognitive impairments, and behavioral challenges through structured, individualized plans.Crisis Intervention & De-escalation: Ability to recognize distress signals and apply appropriate techniques to maintain a safe environment.Regulatory Compliance & Medicaid Waiver Knowledge: Familiarity with NYS TBI and NHTD Medicaid Waiver regulations, ensuring compliance in documentation and program operations.Community Engagement & Inclusion: Experience planning accessible community outings that promote socialization and independent living.Staff Training & Supervision: Ability to mentor staff on behavioral interventions, waiver regulations, and best practices in participant care.Electronic Documentation & Compliance Tracking: Proficiency in Medicaid documentation, participant records, and reporting.Program Development & Activity Planning: Skilled in designing structured day programming that enhances cognitive, social, and independent living skills.Microsoft Office & Data Management: Strong skills in Microsoft Office (Word, Excel, Outlook) and waiver program systems for compliance tracking.Transportation & Accessibility Coordination: Knowledge of mobility support, MAS transportation coordination, and adaptive equipment use.PHYSICAL DEMANDS:
Ability to work in a dynamic, structured day program setting with individuals requiring varying levels of support.Frequent standing, walking, bending, kneeling, reaching, and lifting.Ability to lift and carry up to 50 pounds and provide physical assistance, including transfers, repositioning, adaptive equipment and mobility support for participants.Ability to perform tasks that require fine motor skills and dexterity, such as assisting with personal care or adaptive devices.Occasional exposure to various environmental conditions, including outdoor activities and community outings.WORK CONDITIONS
Some evening or weekend hours may be required for special events, training, or compliance meetings.Ability to coordinate transportation and supervise off-site outings in a variety of community settings.Ability to remain alert and engaged in active supervision throughout shifts.Work may involve potential exposure to bodily fluids, requiring adherence to universal precautions and infection control procedures.This position requires flexibility, attentiveness, and a commitment to ensuring the safety, dignity, and well-being of all individuals receiving care.
