As a Licensed Practical Nurse (LPN) at Crozer, you will be responsible for taking home health /hospice referrals from inside and outside sources, which includes case managers, social workers, physicians and community agencies. You will assess referrals for skilled service eligibility.

Responsibilities

Accurately registering patients into the Matrix Care system Transcribes physicians’ orders in accordance with appropriate standards. Investigate clinical information vis the CKHS EMR to facilitate transition of care. Utilizes available tools such as TEAMS and e-fax.  

Education and Training

Current license to practice Practical Nursing in the Commonwealth of Pennsylvania BLS Certification Home Health experience preferred
