The Medical Social Worker collaborates with themultidisciplinary team to provide input in the development of the plan of carefor those patients requiring social work intervention.  The Medical Social Worker provides medicallynecessary intervention to patients who have social or emotional problems thatare, or are expected to be, impediment(s) to the effective treatment of themedical condition and/or rate of recovery. Additionally, the Medical Social Worker provides these services inaccordance with physician’s orders.

Essential Duties and Responsibilities:

Assesses patient’s psychosocial needs; plans for, evaluates,and modifies interventions to meet the established patient goals Facilitates patient referrals to community agencies whenappropriate Adheres to company policies and procedures while providingservices and preparing/submitting required paperwork Maintains knowledge of the field including Medicare,Medicaid, and other third-party reimbursement sources. Participates in planning and implementing qualityimprovement activities as necessary Provides positive experiences in assisting patients,families, physicians, other departmental employees, referral resources, serviceagencies, and others. Psycho-social assesments Supervisor and Scheduler will work to schedule client appointments in close proximity to minimize travel time and Amergisize efficiency for social workers. Social workers will complete 15-20 assessments per week, assigned by their Clinical Supervisor.  There is no ongoing case management required. Once an assessment is complete, the social worker’s involvement with that client typically ends, unless further information is requested by the State of Maryland.

Minimum Requirements:

Baccalaureate degree in Social Work (BSW) or Master’s degreefrom a school of social work accredited by the Council on Social Work Educationper state/client requirements required Minimum of one (1) year of social work experience in ahealth care environment in the last two years preferred Complies with all relevant professional standards ofpractice  Participation and completion of Amergis’ Competency programwhen applicable Current CPR if applicable TB questionnaire, PPD or chest x-ray if applicable  Current Health certificate (per contract or stateregulation)  Must meet all federal, state and local requirements  Successful completion of new hire training as applicable tojob site  Understand patient confidentiality and HIPAArequirements   Ability to effectively elicit/provide information to andfrom appropriate individuals (including, but not limited to, supervisors,co-workers, clients) via strong communication skills; proficiency in theEnglish language is required   Computer proficiency required  Must be at least 18 years of age  Benefits At Amergis, we firmly believe that our employees are the heartbeat of our organization and we are happy to offer the following benefits: Competitive pay & weekly paychecks Health, dental, vision, and life insurance 401(k) savings plan Awards and recognition programs  *Benefit eligibility is dependent on employment status.  About Amergis Amergis, formerly known as Maxim Healthcare Staffing, has served our clients and communities by connecting people to the work that matters since 1988.
