The role you’ll contribute:
The Licensed Clinical Social Worker is a key member of the healthcare team working with patients and their families to help manage the difficulties of a health or life problem. Utilizes professional social work skills to assess patients for psycho-social and economic barriers that interfere with the attainment of healthcare goals. Works closely with Case Managers, Physicians, Nursing staff, spiritual support and others, as appropriate, to advocate for the patients and families with counseling services, crisis intervention, stress management, patient and parent coping (as appropriate), bereavement, abuse and neglect issues and continuity of care.

Applies knowledge of local, state, and federal programs to assist with appropriate referrals
Actively contributes to the social work program supporting patients, families and physicians
Participates in the development of programs to support families of patients dealing with complex medical issues
Ensures communication of pertinent patient information among caregivers. Effectively utilizes resources in the development of social work programming in AdventHealth and AHMG
Attends physician rounds and core meetings to facilitate communication and clinical social work input.
Qualifications
What You Will Need:
Master’s Degree in Social Work
Licensed Clinical Social Worker
Current BLS/CPR
Experience in ambulatory care social work is preferred
Experience in clinical counseling, including palliative care
Three years of experience in oncology, hematology, palliative care social work
