The Role

The Licensed Clinical Social Worker uses a clinical lens to assess, evaluate and address behavioral health needs of the patient population.  The LCSW will engage with patients through psychosocial assessment, short-term structured intervention and facilitating connection to necessary resources for ongoing support.

 

The Licensed Clinical Social Worker:

Job Functions

·       Conducts comprehensive assessment of behavioral health needs and formulates diagnosis as indicated

·       Employs a variety of strategies, approaches and modalities to address acute behavioral health symptoms, with a focus on CBT and mindfulness techniques

·       Identifies and resolves barriers that hinder effective engagement with care

·       Ensures patient is progressing towards desired outcomes by continuously monitoring symptom progression through structured assessments

·       Serves as a liaison between health care facilities, community resources and government agencies, assisting in fostering relationships with facilities to which patients are frequently admitted

·       Participates in care planning process within multidisciplinary team to collaboratively address patient needs

·       Provides support and guidance to patients and families through advance care planning discussions as indicated

·       Understands department, segment, and organizational strategy and operating objectives, including their linkages to related areas

Required Qualifications

·       Master’s degree in social work (MSW)

·       Clinical Social Work License (LCSW) in Virginia

·       Willingness to obtain licensure in additional states as needed

·       Minimum of 4 years experience working as a Clinical Social Worker

·       Knowledge of community health and social service agencies as well as behavioral health resources

 

Preferred Qualifications

·       Experience working with the geriatric/senior population

·       Bilingual English/Spanish (ability to read, write and speak)

·       Experience conducting diagnostic assessments

·       Experience in crisis management and intervention

Pay Range

The compensation range below reflects a good faith estimate of starting base pay for full time (40 hours per week) employment at the time of posting. The pay range may be higher or lower based on geographic location and individual pay will vary based on demonstrated job related skills, knowledge, experience, education, certifications, etc.


 

$63,400 - $87,400 per year


 

This job is eligible for a bonus incentive plan. This incentive opportunity is based upon company and/or individual performance.
