POSITION SUMMARY

Under the supervision of the Program Director, Licensed Clinician is responsible for the clinical oversight of individuals diagnosed with a variety of mental health and/or substance abuse and/or behavioral conditions.  Licensed Clinician provides crisis intervention, direct support, medication monitoring and oversight, assistance with daily living skills, and any other assistance that individuals need while participating in site-based services.  The team member will assist in the development of the Initial Plans, Relapse Prevention Plans, and Support Plans. Licensed Clinician will also run therapy groups on specific topics, provide individualized supportive counseling, and conduct assessments as needed or required. 

Responsibilities

DUTIES, RESPONSIBILITIES, AND ESSENTIAL FUNCTIONS

Performs assessments to evaluate a variety of mental health and/or substance abuse and/or behavioral conditions to children, adolescents and adults.Develops individualized, prescriptive treatment plans to fit the needs of each patient using appropriate diagnostic and patient placement criteria.Conducts individual, family, and group counseling, utilizing methods consistent with evidence-based practice, using a brief, focal model of care when appropriate.Utilizes outcome tools to determine patient progress.Coordinates patient care with primary care, other specialty providers, schools, and families via written and verbal communication.Arranges for appropriate care within the FREE umbrella or through outside providers as prescribed in policies and procedures.Ensures that each patient’s rights are protected and that they are provided with appropriate interventions based on ability to participate, cultural concerns, and diagnostic impression.Participates in Departmental activities such as clinical care conferences, integrated care committees, risk meetings to ensure the highest quality of care is met.Maintains progress notes and other documentation required so that the client medical record is completed contemporaneously.Advocates for the needs of the individuals in team discussions, communications with collateral organizations, and other contactsParticipates in the review of referral information when applicable, and case reviews as neededEnsures that all required documentation (paper and electronic) is completed /processed appropriately, in a timely manner, and meets quality and compliance thresholds.Performs other duties as assigned.

Qualifications

QUALIFICATIONS

Education:Master’s Degree in human services field requiredExperience:At least 1 (one) year of experience working with outside treatment providers and mental health professionalsSpecialized Training, Licenses, Certifications:Licensure as either an LMSW, LCSW, or LMHC requiredRight candidate with no licensure might be considered, but would need to sign educational agreement and would only qualify for entry level 1 on salary scaleKnowledge, Skills, Competencies, and Abilities:Bilingual – for specific bilingual roles and language may differ based on the needComputer literacy requiredDemonstrated ability to work in a team environment with flexibility, keeping others informed and anticipating needs is essentialMust be efficient, well organized, and have excellent communication skillsCritical thinking and sound judgment skills are necessaryOther Requirements:Valid NYS Driver’s License and satisfy criteria to be a driver for FREE

WORK ENVIRONMENT

This role is performed in-person. Some travel is required based on the case load.

PHYSICAL REQUIREMENTS

Employee is required to sit, walk and stand for prolonged period of time (at least 8 hours).Employee is required to effectively communicate both verbally and in writing.Employee is required to operate standard office equipment such as computers, phones, photocopiers, fax and filing cabinets.Employee is required to reach with hands and arms.Employee might be required to move and/or lift light items.

The Pay Range for this job is $61,000 - $68,000. This range is based on what FREE reasonably believes it will pay an employee as for said position at the time of the posting. Individual compensation will ultimately be determined based on a variety of relevant factors, including, but not limited to, an individual’s experience, qualifications, services provided, geographic location, relevant skills, and other factors permitted under federal, state, and local laws.
