The BayCare Population Health Service Organization (PHSO) department is part of a collaborative practice model that includes BPP clinicians and office staff, patients, caregivers, Enterprise Care Coordination Office (ECCO), Value Based Care (VBC) total care management strategies, and the community. The BayCare ECCO program is comprised of Nurse Care Partners, Social Service Specialists, Pharmacists and Licensed Counselors who provide care coordination and motivational interviewing techniques to help support patients beyond their medical conditions and diagnoses. The ECCO team works collaboratively with BPP and assists members with community resource navigation by researching, finding, and linking them to resources, services, and support mechanisms for their care plan and self-management needs. This team member will develop and maintain relationships with VBC members, acting as the contact point to assist with health and social needs in collaboration and partnership with the clinicians and ECCO’s other clinical programs.
