Key Responsibilities:

Provide individual and family counseling to support mental health and emotional well-being. Develop personalized treatment, discharge, and aftercare plans. Collaborate with referring agencies, community resources, and multidisciplinary teams to ensure comprehensive care. Maintain accurate and up-to-date treatment records. Use your expertise to assess and address the unique needs of consumers across all age groups, providing appropriate, age-related care.

 

What We’re Looking For:

A licensed professional (LSW, LAC, LPC, or LCSW) with a strong understanding of therapeutic practices. Someone with the skills to assess consumer needs and deliver tailored care through counseling and psychotherapy.
