Key Responsibilities : + Provide individual and family counseling to support mental health and emotional well-being. + Develop personalized treatment, discharge, and aftercare plans. + Collaborate with referring agencies, community resources, and multidisciplinary teams to ensure comprehensive care. + Maintain accurate and up-to-date treatment records. + Use your expertise to assess and address the unique needs of consumers across all age groups, providing appropriate, age-related care. Skills and Requirements : + Strong communication and interpersonal skills. + Must possess a valid driver’s license. If duties involve driving, the ability to operate an agency vehicle is required.
