the coordination of care for patients presenting with behavioral health and substance abuse needs while working collaboratively with interdisciplinary teams Responds to crisis calls Conducts advocacy referral and discharge planning to meet patientfamily needs Works with patientsrsquo community behavioral health and medical providers to assure coordination of clinical care Facilitates treatment
