Duties Description As a Licensed Master Social Worker at Elmira Psychiatric Center, you will be part of a multidisciplinary team that provides culturally competent and evidence-based treatment and support services in a recovery-oriented environment to individual with mental illness. Duties will also include: Providing crisis intervention, service linkages, discharge planning, facilitating groups and individual sessions• Developing treatment plans with individuals and treatment team; monitoring and evaluating progress; and modifying treatment plans as needed• Determining discharge needs and appropriate community resources• Establishing and maintaining relationships with community providers to ensure continuity of careNOTE:1.
