POSITION STANDARDS High school diploma required. New Hampshire LNA license required. Medical terminology required. One year of previous experience working in a medical setting, or the equivalent preferred. One year of previous experience as an LNA in an acute care setting preferred. Detail oriented with the ability to multitask. Must be able to work independently as well as in a team setting. Strong organizational skills. Strong computer skills. Strong customer service skills. Strong Microsoft Office skills. BCLS preferred on hire; required within 30 days of hire. Position Objective As part of the Nursing team, and at the direction of the Nurse Manager, is integral in supporting patient and unit flow.

 

Performance Expectation Health Unit Coordinator Performance Expectations Communicates with other HUCs to complete general secretarial duties to facilitate a well-run and organized department Manages department phone calls and messages to support clear and accurate multidirectional communication Greets anyone entering the department to support a welcoming environment and to provide direction, answer questions or re-direct as appropriate Manages appropriate levels of office supplies in a fiscally conscientious manner to support the staff directly caring for the patients Supports timely, accurate, and confidential flow of patient documentation through chart preparation, faxing, filing, copying and verification of completeness Supports timely and accurate management of providers’ orders by communicating in the electronic medical record and all other supporting documentation (e.g. signing off with date & time, updating in e-DH etc.) Enters department work orders to the appropriate departments in a timely manner and follows up as necessary for completionDoes charge entry timely and accurately to sustain profitability within the department Maintain equipment manuals and product pamphlets to ensure information is easily obtained and useable by the clinical staff (including equipment weight limits) Provides general support to providers for the unit’s patient care needs (e.g. scheduling patient follow up appointments, arranging transportation to approved clinical department appointments, notifying providers of consult requests, finding additional patient information as requested by provider etc.) Provides general support to the charge nurse to maintain clear multidirectional communication amongst staff members in a timely manner Assists clinical staff on the unit to best meet patient needs (e.g. answer call bell lights, assist with stretchers, obtain equipment like wheelchairs, coordinate and assist with transportation to another department within the hospital, etc.) Assists Nurse Manager with general unit projects as directed LNA Performance Expectations Sets up rooms for new admission and post-op patients Performs Accu-check controls when indicated Makes beds and distributes meal trays and maintains clean environment in patient rooms Performs vital signs as scheduled on patient, charts on flow sheets within e-DH and reports any changes to the Nursing staff Provides clean linen and fresh water to patient twice a day, or more often if needed Provides AM/PM care/bathing as instructed by nursing staff and documents task within electronic medical record Record accurate I + O's on patients flow sheets within electronic medical record on all shifts, empties Foley catheters at end of shift and reports any changes to Nursing staff Assists nursing with wound care as directed Assist PT/OT with patient transfers and ambulating Answer patient call lights in a timely way, alerting Nursing Staff of patient needs Perform Accu-checks on patients as ordered, notifying Nursing of results and charting Performs other related duties as directed by the Nurse Manager of Medical Surgical Services Demonstrates unit specific competencies regarding
