As part of the community-based outpatient home care program, the Paramedic conducts home visits and provides treatment for chronically and acutely ill, high-risk patients. All visits and treatments are conducted under the close guidance and orders of the aligned, licensed Provider and Provider Leadership. Responsibilities include assessment and triage, conducting medication reconciliations and reinforcing discharge and medical treatment plans. Assumes ownership and leads advanced and highly specialized operational/customer support duties that require independent initiative and judgement.
Essential Duties and Responsibilities:
Careful examination of patient for signs and symptoms of illness or injury.Vital signs assessment.Timely documentation of patient condition with information relayed to provider.Execution of provider orders while at the home to include:Administration of medications Oral, injectable, IV, or inhalation routeWound care and dressing changeEKGLab drawsAdministration of IV fluids and/or IV vitaminsVaccinationsAssessment of patient’s home for fall risks, environmental triggers for Asthma patients and general home safety.Demonstrate effective communication skills with patients and their families, center staff and providers.Facilitate TeleHealth Visits with the providers while in the home to ensure patient is engaged in their care and treatment plan(s).Conduct a medication reconciliation with the patient.Assess and triage post discharge patients that are not able to go to provider office or complete a telehealth visit independently.Reinforce discharge plans and medical treatment plans.Deliver customer-oriented treatment and support to high-acuity patients.Ability to set goals and work independently in the field while escalating important medical information to the provider and center staff with an end-goal of reducing total expenditures and maintaining the patient at home.Licenses/Certifications/Requirements: (Valid and in good standing):
Certification in Basic First AidActive Paramedic LicenseFlorida Driver’s License in good standingActive CPR CertificationCertification in Basic Life Support (BLS) for Healthcare ProvidersAt least 1 year or more of experience as a ParamedicPreferred qualifications
