LPN’s provides and directs the provision of nursing care during shift, based on Fox Run policies and procedures, through the competent application of the nursing process.  Will provide and supervise the daily nursing care of Residents.

RESPONSIBILITIES:

Administers and documents medications, treatments, and other nursing services. Plans and organizes activities to ensure resident needs are met in accordance with instructions of physician, Fox Run policy and regulatory mandates. Evaluates the nursing needs of the residents; initiates, develops, implements and makes necessary revisions to the resident’s Individual Service Plan (ISP). Develops, prepares and maintains individualized resident care progress records with accuracy, timeliness and according to policies. Responsible for following all policies and Procedures regarding care delivery, documentation and care coordination. Practices and teaches standard universal precautions. Supervises and teaches the Personal Care Aides and other direct care staff.

QUALIFICATIONS:

Degree in nursing from a program accredited by NYS school of nursing.  Licensed as a Practical Nurse by the state. Minimum of one year of nursing experience required. Knowledge and Abilities:
a) Demonstrated knowledge and skills necessary to provide care to and communicate with primarily the geriatric population.
b) Demonstrated knowledge of the principles of growth and development over the life span.
c) Able to assess data reflecting the resident’s status and interpret the appropriate information needed to identify each resident’s requirements relative to their age-specific needs.
d) Able to use computer, medical software, and word processing programs.

Hourly range: $24.50-$26.00 (depends on experience)
