SUMMARY

The Licensed Practical Nurse (LPN) assists each resident in achieving and maintaining his or her highest practical level of physical, mental, and psychosocial well-being through providing medication, treatment administration and quality, personal care


PRIMARY RESPONSIBILITIES

Provide direct care to residents under individual plans of care, administering care and medications per doctor’s orders.*Act as an advocate to maintain resident privacy and dignity during cares. Administer prescribed medications and treatments per doctor’s orders. *Monitor resident vitals, completing resident focused assessments as assigned.    Communicate any identified changes to RN Nurse Manger/LPN Clinical Coordinator, Clinical Lead, medical provider and family members as instructed. *Collaborate with the IDT (interdisciplinary team) to promote adequate and appropriate flow of services.*Monitor dining room activities, assisting residents with eating as needed.Assist throughout the admission process, to ensure smooth transition of new residents. *Periodically perform focused assessment of residents with acute changes of condition reporting changes to the medical provider.Attend resident Care Conferences as needed.Monitor and provide guidance to NARs, and TMAs. *Communicate expectations and monitor that unit personnel are carrying out their assignment. *Organize and provide work assignment to unit staff. *Collaborate with staff regarding nursing care of residents. Provide in-service education as requested from nursing administration. *Maintain and complete all required documentation and charting on the computer or on forms in an accurate and timely manner according to policies and procedures. * Accurately chart Nurse’s notes reflecting the care provided and resident response. *Initial Medication Administration Record (MAR) after medication is administered per facility policy and procedure, accurately documenting refusals for medication on back of MAR. *Support various other Nursing responsibilities as needed.

JOB SPECIFICATIONS

Education, Experience and Credentials

Must be licensed as a License Practical Nurse in the State of Minnesota and must maintain licensure.One or more years experience in long-term care highly preferred. Current certification in CPR required.

Knowledge, Skills and Abilities  

Flexible and productive in an environment with regular interruptions.Display high levels of customer service, responding promptly and thoroughly to the inquiries and needs of individuals both internal and external to the organization.Possess effective problem solving skills.Strong computer skills, including Microsoft Word, Excel, Outlook, Internet, HRIS, and other applications.  Must be skilled in database management and record keeping.Understand and maintain confidential nature of organization and client information in accordance to HIPAA regulations and corporate compliance standards, including appropriate access of data housed within the Information Technology infrastructure. Able to display high levels of customer service, responding promptly and thoroughly to the inquiries and needs of individuals both internal and external to the organization in accordance with our PROUD standards.Able to understand and follow facility and department safety and emergency procedures.

 

PHYSICAL REQUIREMENTS NECESSARY TO PERFORM THIS JOB

Incumbent must be able to:

Perform heavy work, supporting up to 100 pounds, lifting up to 50 pounds with assistance, with occasional lifting/carrying of objects weighing up to 25 pounds.Effectively communicate in the English language, including following oral and written instructions, in order to communicate with people inside and outside the organization.Work at a desk for extended periods and perform work at a computer. Physical ability to move around the department and throughout the facility repeatedly throughout the day.Apply manual dexterity and for computer keyboarding and medical and office equipment use.Vision acuity to read fine print, warning labels, doctor prescriptions, etc.Turn, bend, stoop, kneel, squat, and reach frequently in the performance of nursing responsibilities.Work a flexible work schedule, including overtime, nights and weekends as required including attending events/meetings as needed.

 

TOOLS AND EQUIPMENT USED

Use thermometer, stethoscope, blood pressure cuff, wheelchair, patient lifts, and related medical equipment.Use telephone, computer and standard office equipment. 

 

WORK ENVIRONMENT

The work environment is indoors. The incumbent must be flexible about switching positions quickly in order to respond to the needs of residents, in responding to emergencies.   May be at risk of exposure to blood, bodily fluids, and other potentially infectious material.
