We are seeking a professional, compassionate, and energetic Licensed Practical Nurse to be a part of our Fellowship Square, Home Health Team! The individual will be dedicated to enriching the lives of our senior residents and committed to meeting their day-to-day needs while helping to preserve their dignity and quality of life.

 

The LPN shall provide skilled nursing services to various Home Health patients under the direction of the RN Case Manager or Director of Nursing and follow physician orders and HHA policy and procedures.

Currently needing coverage for the following locations: 

One LPN to cover the northwest & central Phoenix location.One LPN to cover the southeast Phoenix location.  Responsibilities Must be in compliance with the State of AZ scope of Practice and Nurse Practice act.Complete mandatory CEU’s to maintain LPN licensure.Follows specific physician orders for the administration of care as directed on the 485 plan of care.Ability to follow philosophy, objectives, policies, and procedures that enhance high standards of skilled nursing practices.Keeps abreast of new knowledge and information so that clients will be assured of receiving up‑to‑date methods of care.Participates in formal continuing education sessions and in-service sessions.Maintains open relationships and communication with client’s, client’s families, physicians, and other HHA staff involved in the care of the client.Follows all State and Federal rules and regulations as they pertain to the Home Health Center.Is responsible for following Infection Control Manual regulations.Responsible for an active role in making sure the rehabilitation program is directed towards assisting each client on their case load to achieve and maintain the highest level of self‑care and independence as recorded in the client's care plan.Documents each home health visit within the medical record to include on the nurse's notes significant observations regarding the resident's needs for and/or response to medications/treatment.May be assigned other duties and responsibilities by the RN Case Manager or DON.  Qualifications Must be a graduate of an approved school for licensed practical nursing or registered nursing. Must hold a current, valid LPN license in the State of Arizona.Minimum of 2 years' experience.Must have an understanding of DHS rules. Previous experience in a geriatric setting preferred. Must be flexible and have a desire to communicate and work with seniors. Knowledge of Home Health rules and regulations required.Must be motivated, self-starter, with a passion to serve seniors.
