Job Description: Job Title: Licensed Practical Nurse (LPN) Location: Hannibal, MO Department: Primary Care Schedule: Monday - Friday, 8 am - 5 pm Job Summary: The Licensed Practical Nurse (LPN) is responsible for providing nursing services to patients in residential care facilities, behavioral health, and/or substance use disorder treatment programs. The LPN works to maintain a safe and therapeutic environment for clients and provides support through various nursing duties across outpatient psychiatry, addiction, and housing services. Additionally, they work to build and maintain a strong working relationship between clients and clinic staff. Position Perks and Benefits: + Competitive hourly rate + Opportunities for professional growth and advancement + Friendly and supportive work environment + Flexible scheduling + Professional development opportunities + Recognition and reward programs + Paid time off and holidays + Health, dental, and vision insurance + 401(k) plan with company match Key Responsibilities: + Administer medications to clients via various methods, including oral, injection, self-administration, and others, according to service line requirements. + Perform nursing assessments, including physical and mental health assessments, and psychiatric screeners, in line with service line requirements. + Document medication administration and nursing assessments accurately, adhering to service line standards. + Conduct Medication Administration Record (MAR) reviews and medication counts, monitoring the house stock of medications and refill status for housing clients. Prepare to pass medications as needed. + Communicate with clients regarding medication questions and needs, staging refill requests to providers in accordance with standard operating policies and procedures. + Room clients, verify client data and medical history, confirm the purpose of visit, and obtain necessary physical preliminary requirements such as blood draws, urine samples, height, weight, blood pressure, pulse, and temperature. + Complete all required documentation in a timely manner, including nursing assessments, physical and spoken interventions with clients, nursing-led educational groups, and client safety plans. + Ensure that all medical and medication records are up-to-date and accurate, assisting with general lab and phlebotomy duties as needed. + Function as a resource person for nursing and medical issues, overseeing infectious disease control, client hygiene, and related client and facility health issues, under the supervision of nursing leadership. + Participate in all required meetings as assigned, and perform other related duties as required, assigned, and/or delegated by nursing leadership. Knowledge, Skills, and Abilities: + Proficient in accurately entering, transcribing, recording, and storing information in various formats, including written and electronic forms. + Able to assess and provide feedback on programs and services to improve their effectiveness and efficiency. + Strong communication skills, both verbal and written, able to produce clear and concise reports and convey complex ideas in a simple and respectful manner. + Committed to maintaining confidentiality and protecting sensitive information, including medical records. + Adept at reading and interpreting various types of documents, including laboratory results, notes, and electronic health records, as well as safety rules, operating instructions, and procedural manuals. + Skilled at writing routine reports and correspondence that are clear, effective, and respectful, using multiple modes of communication, including in-person, phone, and video conferencing. + Competent in basic mathematical calculations, including addition, subtraction, multiplication, and division of whole numbers, fractions, and decimals. + Possesses strong problem-solving abilities, capable of understanding and following instructions, and adapting to new situations and challenges. + Demonstrates a willingness to learn, grow, and work collaboratively with others, embracing diverse perspectives and approaches. Experience and Education Qualifications: + Current licensure as a Licensed Practical Nurse (LPN) in the state of practice. + Graduation from an accredited practical nurse program. Supervisory Requirements: + N/A Employment Requirements: + Successful completion of a background check, including criminal record, driving record, abuse/neglect, and fingerprint check. + Completion of New Hire Orientation at the beginning of employment. + Fulfillment of all training requirements, including Relias Learning at the beginning of employment and annually thereafter. + Current driver’s license with an acceptable driving record and current auto insurance. Physical Requirements:ADA Consideration - Sedentary work: Exerting up to 10 pounds of force occasionally (up to 1/3 of the time) and/or a negligible amount of force frequently (1/3 to 2/3 of the time) to lift, carry, push, or pull objects, including the human body. Repetitive movements of hands, fingers, and arms for typing and/or writing during work shifts are common. Sedentary work involves sitting most of the time but may involve walking or standing for brief periods.
