Job Title: Licensed Practical Nurse (LPN)
Job Description

Provides professional nursing care to patients and their significant others following established standards and practices. The LPN coordinates with other members of the care team to ensure seamless care delivery, maximal coordination of efforts, and active patient participation in planning and care. Assists physicians or other providers, performs telephone triage as needed, and participates in patient teaching.

ResponsibilitiesProvide professional nursing care to patients following established standards and practices.Coordinate with other members of the care team to ensure seamless care delivery and maximal coordination of efforts.Encourage and facilitate active patient participation in planning and care.Assist physicians or other providers with various tasks.Perform telephone triage as needed.Participate in patient education and teaching.Conduct clinical questionnaire assessments.Essential SkillsKnowledge of medical terminology.Experience with electronic medical records (EMR).Case management skills.Current New York State (NYS) license preferred.Proficiency in triage and answering phones.Experience in family medicine, internal medicine, pediatric care, and flu clinics.Familiarity with NextGen and Epic healthcare systems.Additional Skills & QualificationsGraduate of an accredited school of nursing.Licensed to practice in New York State (NYS) with current NYS registration and in good standing.Record of prior stable employment and/or favorable references.Work Environment

Requires full range of body motion including moving and lifting patients, manual and finger dexterity, and eye-hand coordination. Requires sitting and/or standing for extended periods of time. Work locations available in Mount Kisco, Poughkeepsie, Rhinebeck, Carmel, Yorktown, and Mount Kisco. No holiday or vacation time offered, but sick time is available.
