JOB TITLE: Licensed Practical Nurse Shift: Full-Time, Part-Time, days, evenings, & nights, flexible scheduling! PAY RATES FOR THIS POSITION HAVE RECENTLY INCREASED: Now Starting at $26 - $30.50/ hour Position Scope: Under the direction of the Unit Manager/Supervisor, will assist with day to day clinical management, medication administration, and treatment administration. Qualifications/Education/Experience: + Graduate of an accredited nursing curriculum + Current New York State Licensed Practical Nurse (LPN) Responsibilities: + Provide, assist, and monitor other team members to provide high quality nursing care using a holistic and person centered approach. + Be sensitive to the changing and ongoing needs of residents, families, and other caregivers. + Monitor care delivered to ensure the plan of care is being upheld and remains appropriate to resident needs. + Follows facility policy and procedure for clinical and infection control practices. + Liaise with medical and other members of the interdisciplinary team to ensure quality care. Provide support to the Unit Manager/Supervisor, bringing to their attention any matters they should be aware of e.g., accident and incidents, change in resident condition, etc. + Contribute to the implementation and ongoing adherence to clinical protocols and standards of care. + Provide therapeutic treatments and procedures as ordered by medical staff e.g., medication administration, wound care, obtaining lab specimens, etc. + Update the appropriate medical staff with change in resident condition, laboratory results, resident/family concerns, etc. + Update family/responsible party with change in resident conditions, incident and accidents, etc. + Follow up with acutely ill residents, residents with a recent incident, and those with change in condition on a daily basis to ensure improvement. If decline is noted update medical and Supervisor. + Communicates and collaborates with other members of the interdisciplinary team to maintain quality care. + Participate in Team Member meetings. + Maintain medical records in accordance with facility policy. + Assist in maintaining unit equipment, supplies, and inventory. + Promptly report equipment/maintenance problems to relevant individuals. + Attends in-services to keep current, knowledgeable, and remain compliant with requirement of mandatories. + Other duties as assigned and within scope of practice. HIPAA Compliance : Demonstrates knowledge of and adheres to LCCN policies and HIPAA regulations governing the confidentiality and security of protected health information for both manual and electronic resident records, charts, information and other confidential information including (but not limited to) employee records, computer access codes and passwords. Physical and Sensory Requirements: (With or without the aid of mechanical devices) + Ability to communicate with residents and families, inter-departmental, nursing team members, visitors, government agencies, and the general public. + Must possess the ability to make independent decisions when circumstances warrant such action. + Must possess the ability to seek out new methods and principles and be willing to incorporate them into existing practices. + Must be able to move intermittently throughout the workday, including walking, standing, sitting, with ability to lift, push, pull, and move a minimum of twenty five (25) pounds. + Must be able to speak and write the English language in an understandable manner. + Must be able to cope with the mental and emotional stress of this position. + Must function independently and have flexibility, personal integrity, and the ability to work effectively with residents, staff and support agencies. + Must be in good general health and demonstrate emotional stability. + Must be able to relate to and work with ill, disabled, elderly, emotionally upset and at times, angry people within the facility. + Must be able to assist in the evacuation of residents.
