Job Duties
PATIENT CARE
Collects data from patient, family or significant others and records on Initial Data Base form.
Contributes to the formulation and updating of plan of care and implements plan of care under the direction of the Registered Nurse.
Gives direct nursing care to assigned patients (e.g., medications, treatments, dressing changes, TPR) according to established standards.
Recognizes changes in patient condition and responds appropriately, notifying charge nurse as needed.
Documents pertinent information regarding patient's responses and nursing interventions in patient's medical record.
Administers medication according to hospital policies.
PATIENT CARE RELATED DUTIES
Applies principles of infection control and safety in the performance of patient care.
Communicates effectively with charge nurse and other member of the Health Care Team regarding patient's needs and/or changes in condition.
Responds appropriately in crisis situation.
Reports significant unit problems to the R.N.
Patient care responsibilities are delivered with a knowledge of patient growth and development and are appropriate to the ages of the patients served.
Customer Service: respect, flexibility, knowledge, confidence, professionalism, pleasant attitude, patience and helpfulness. All responses should be timely, professional, caring, and respectful in accordance with Customer Service Performance expectations
Maintains established departmental policies and procedures, objectives, quality assurance program, safety, environmental, and infection control standards. Assesses gaps in policies and procedures, and create necessary policies and procedures to fulfill these gaps.
Understands and adheres to the Medical Center’s Code of Conduct.
Familiar with the Medical Center’s Mission, Vision, and Values Statements.
OTHER JOB DUTIES
Orients patient and family to new environment; develops rapport with patients family and/or significant others.
Orients LPNs, Nurses’ Aides and orderlies as required.
Effectively utilizes the hospital computer system as it pertains to job responsibilities and in accordance with hospital policy.
Participates on unit committees and in CQI activities as assigned.
Participates in patient care conferences and in staff development activities as required.
Assists other personnel as needed.
Performs other related duties as required.
BASIC COMPETENCIES
Education/Licensure
Graduation from an accredited school of Practical Nursing or equivalent education as described by the State Education Law.
Current NJ license as an LPN.
Successful completion of Progressive Care Orientation Program required.
Experience

Skills
American Heart Association Basic Life Support certification required.
Clinical nursing skills appropriate for an LPN.
Good oral and written communication skills.
Good interpersonal skills.
Speaks, reads and writes English to the extent required by the position.
JOB SETTING/PHYSICAL DEMANDS
All patient care areas; contact with patients, visitors, staff.
Frequent prolonged standing/walking.
Lifts, positions, pushes, and/or transfers patients.
Protective clothing worn: face shield, gloves, impermeable gown, and particulate respirator
