Your Responsibilities as an LPN Clinical Coordinator

In this role, you’ll serve as the backbone of patient care coordination, facilitating smooth communication and optimal care outcomes for patients and their families. Key responsibilities include:

Care Coordination: Maintain accuracy in documentation through Homecare Homebase (HCHB) to ensure all workflow standards are met.

Communication Bridge: Serve as the main connection between patients, CTMs, medical staff, and facilities, enhancing communication and care coordination.

Patient Advocacy: Assess and respond to patient needs, advocating on their behalf to ensure the highest quality of care and compassion.

Decision-Making & Problem Solving: Use sound judgment to make decisions, manage complex situations, and escalate issues when necessary.

Conflict Resolution: Address and resolve complaints effectively to ensure positive outcomes for patients and staff.

Leadership & Team Support: Foster a collaborative work environment, offering guidance and support to help your team thrive.

What You’ll Need to Succeed

Licensed Practical Nurse (LPN): Graduate of an accredited nursing school with an active LPN certification.

Relevant Experience: 1+ year of practical experience, ideally in Home Health or Hospice care.

Strong Communication Skills: Ability to communicate clearly, actively listen, and adapt messaging as needed.

Time Management Skills: Organized, efficient, and able to handle multiple priorities.

Tech-Savvy: Proficient with systems like Homecare Homebase, Workday, and Microsoft Office.

Patient-Centered Focus: Skilled in understanding and addressing patient needs, with a commitment to advocating on their behalf.

Bilingual (Preferred): Proficiency in Spanish is a plus.

Valid Driver’s License & Reliable Transportation: Required for travel in support of field duties.
