Under the Supervision of the Assistant Director of Nursing, the Licensed Practical Nurse (LPN) is responsible for the overall oversight and supervision of the healthcare needs of the people we support in a residential setting. The LPN will work in conjunction with the Assistant Director of Nursing, Registered Nurse and the Residential Administrative team to ensure the delivery of the highest quality of integrated health and continuity of care to the people residing in our residences. The qualified candidate must be able to work in partnership with all personnel within the organization to ensure the highest quality of clinical services for the people we support in a manner that is consistent and aligned with the agency mission and vision, and the philosophy of the Council on Quality and Leadership (CQL).

Responsibilities Perform delegated nursing functions within the scope of practice of an LPN under OPWDD (See attached guidance) and as per established procedures, policies, guidelines and standards as observed by the Registered Nurse. Assist in data collection on individuals within assigned caseload including, but not limited to, lab results, diagnostic testing, BM data.Perform tasks and responsibilities related to individual’s medical care and environment in a safe manner. Contribute to the development of individual’s plans and assist in identifying areas of need. Adhere to physician orders accurately. Assist in observation and evaluation of nursing care administered by Direct Support Professional Team Members.Document pertinent observation and intervention as required by policy and regulation: including, but not limited to, AMAP observations, BM protocols, feeding protocols, diabetic protocols.Assist in maintaining effective communication with staff, physicians and Registered Nurse. Communicate information regarding individual’s medical status and overall needs to the Registered Nurse, physicians and pertinent team members. Qualifications New York State Licensed Practical License.Experience working with the differently-abled population preferred.Knowledge of OPWDD state and federal regulations preferred.Ability to manage multiple priorities and ability to be flexible to meet the needs of a diverse populationStrong verbal and written communication and organizational skills. Valid NYS Driver’s License and meets the criteria to be a valid agency driver.
