Role: Licensed Practical Nurse (LPN)

Team: Tech-Enabled Managed Services (TEMS) - Ambulatory Clinics

Location: Thibodeaux, LA

Travel: none anticipated

PRIMARY PURPOSE/FUNCTION:

The LPN will perform a variety of licensed professional nursing duties to provide patient care in support of the physicians and/or other medical personnel in the practice.  The LPN will provide a positive patient experience for the patients. 

PRIMARY RESPONSIBILITIES:

Clinical Functions

Greets all patients and visitors with courteous, pleasant manner.Answers telephone in courteous manner taking messages or directing calls as needed.Assists Physician with appointment book including patient appointments, follow up visits, and/or procedure appointments.Schedules ancillary services as needed. Obtains necessary authorizations from payers for outpatient or inpatient services.Assists patients with referral appointments as needed.Interview patient and collect clinical data elements.Assist providers with obtaining quality data from the patients that will be used by CMS or other payers to measure quality performance. Also, entering the data obtained accurately into the EMR.Performs nursing assessment by obtaining and documenting medication list, allergies, vital signs, patient complaint, and history & physical in the patient chart.  Communicate concerns and/or problems regarding the patient to the provider. Remain available for the provider to assist with clinical services.Assist provider in patient rooms, if needed. Review and complete provider orders, give injections, and schedule tests according to provider protocol.Create and maintain accurate and complete documentation including the patient medication list, allergies, chart notes and detail documentation regarding phone calls into the medical record. Communicate with other staff to assist with patient flow and document patient wait times.Respond to phone calls such as: pharmacy refills, patients, provider offices, home health agencies, etc.Perform blood work (veni-punctures & finger sticks) and/or assist with duties associated with the office visit.Provides education to patients related to practice specialty.Explain diagnostic procedures to patient (such as: CT, MRI, Holter monitors, event recorders, EKG, stress test, treadmills, ultrasounds, nuclear medicine treadmills, etc). Provide patient and family education when needed.Assist as needed with filling out forms such as home health requests, attending provider statements, employee medical leave forms, work/school doctors excuses, handicap license, etc.May perform as a scribe for the provider.Assist in the ordering of medical supplies. Assist with all mandated quality clinic checks.Review patient satisfaction results that directly reflect the nursing questions of the provider or specialty.Take personal accountability for the patient satisfaction scores that are directly related to this position.Offer process improvement suggestions to increase the patient satisfaction scores to the 99th percentile.Performs other related duties as assigned by Supervisor.

QUALIFICATIONS:

Educational and/or Professional Qualifications:

Graduate of a LPN program required.

Previous Experience:

1.  Must possess good interpersonal, communication and organization skills.

Licensure, Certification, Registration, etc.:

1.  Valid license in State of Louisiana as an LPN required.

SUPERVISION EXERCISED:

No direct line of supervision.

CONTACTS INSIDE AND OUTSIDE THE ORGANIZATION:

Daily contact with Supervisors/ManagersDaily contact with Patients/Family membersDaily contact with Co-workersDaily contact with Providers

EQUIPMENT, MATERIALS, AND VALUABLES:

Patient care equipment.Computers and other office equipment.

SCHEDULE:

Typical work hours are Monday through Friday from 8:00 a.m. to 5:00 p.m.   Adjustments to schedule may be made to meet patients’ needs.

HAZARDS:

Job duties will include tasks which have the potential for daily exposure to patient blood/body fluids.Exposure to sharp objects/latex.

PHYSICAL EFFORT:

[Note: In terms of an 8/12 hour day (shift length), “Occasionally” equals 1-33%, “Frequently” equals 34-66%, and “Continuously” equals 67-100% of the time].

Intermittent sitting and standing.Constant walking.Frequent lifting, carrying, pushing, pulling, bending, squatting, kneeling, reaching, and twisting.Must be able to see (near and far), hear, talk, have use of right and left hands, have use of both feet for repetitive movement, have dexterity in fingers.

Information Security and Compliance Responsibilities:

Maintain compliance with training directives required by the organization pertaining to Information Security, Acceptable Use Policy and HIPAA Privacy and Security.Adhere to and comply with the organizations Acceptable Use Policy.Safeguard information system assets by identifying and reporting potential and actual security events to the organizations Security and Compliance Officers.
