ResCare Community Living

Overview

Plans and manages patient care according to each patient's needs. Interviews patients and records their medical history and physical condition. Obtains patient vital signs, including pulse, blood pressure, temperature, and respiration. Provides routine care for patients.

Responsibilities Provide 1:1 care for client always following client’s plan of care while maintaining client's dignity and independenceMonitoring clients’ vital statistics and reporting abnormal findings to the case manager and physician assessmentsPerforms medication administration, feedings, oxygen, ostomy care as needed per client's plan of care and provided assessments of resultsComplete routines as established by other care professionals such as Physical Therapist, Occupational Therapist, and Speech TherapistEnsures infection control policies are maintained at all times, reports possible communicable diseases or infections, and ensures the overall implementation and practice of infection control procedures are implementedAccurately documents nursing actions of all care given and communicates with family, case manager, and physicians as well as documents the progress and outcomes for established goals and informs physician, case manager, and families of changes in client's medical condition and needsOrders supplies and medications, as needed, to ensure adequate amounts are on hand at all timesEnsures all patient needs are being met in accordance with the instructions of the physician.Monitors incidents involving individuals. Ensures incident reports are completed timely, accurately, and case managers are informed; examines the individual following the incidentEnsures all medical equipment is maintained and reports faulty equipment to Durable Medical Equipment for repairOther duties as assigned Qualifications Completed post-secondary coursework to facilitate licensure as a Licensed Practical Nurse or Licensed Vocational NurseKnowledge of individual service plans, including understanding of client rights and health needsProof of current license as a Licensed Practical Nurse or Licensed Vocational Nurse in good standing from the state(s) of practiceCPR CertifiedExcellent interpersonal skills, including customer service, patience, empathy, and compassionIn-depth understanding of patient care policies and regulationsFamiliarity with symptoms and signs of improvement for several medical conditionsModerate amount of travel between facilities may be required About our Line of Business ResCare Community Living has four decades of experience in the disability services field, providing support to individuals who need assistance with daily living due to an intellectual, developmental or cognitive disability.
