Job Responsibilities:
Supervised the clinic activities to ensure that encounters occur in a timely fashion.Monitor and track cases to meet timeframes of regulatory agenciesObtain and document vital signs including: head circumference, pulse, respiratory rate, height and weight, blood pressure, temperature; perform venipuncture and collect specimens for various labs; perform vision and hearing screening; perform skin testing for tuberculosis; administer immunizations as ordered by MD/PNP.Assist MD and/or PNP in conducting physical examinations, set up appointments for referralsFollow-up and obtain reports.Provide any necessary counseling and handouts to children and families.Must demonstrate computer literacy, create and maintain a medical record for each child/youth and complete all documentation in the Electronic Health Record.Ensure that the Electronic Health Record contains documentation of all health services received.Ensure that appointments for recommended health services with off-site providers are made and that reports from the encounters with off-site providers are obtained in a timely manner, reviewed by the agency health care provider and scanned in the medical record. The reason for missed appointments must be documented and missed appointments must be rescheduled.Work closely with social work staff to assure that required consents for routine/emergency care, preplanned medical procedures and psychotropic medication are obtained within mandated timeframes.Assess and triage sick visits.Assist in quality assurance activities involving services provided to children in assigned caseload.Ensure that children/youth receive all required and necessary health and dental services within regulatory timeframes.Coordinate dental and optometry sessions with on-site consultants and ensure that all children in caseload have semiannual dental visits and at least one optometry evaluation, with on-going optometry follow-up as needed.Provide nurse screenings when needed and document results.Ensure that medical records contain documentation of all required adolescent risk assessments and STD risk reduction/family planning counseling.Enter health information data onto data entry forms as required and forward to IT specialist for data entry in a timely manner.Interface with Vaccine for Children Program and NYCIR. Ensure vaccines are appropriately stored and that information on all vaccines administered, is entered into the NYCIR and the EHR on a timely basis. Obtain immunization information from the child’s last known school when necessary.Complete camp, WIC, school and any other medical forms for children in assigned caseload.Monitor health and mental health care information and ensure ongoing case management. Maintain MATS audit review tool in an excel data base.Order and maintain required supplies and equipment.Attend FTC’s and other meetings when needed to provide health related information, which is germane to developing and implementing permanency plan for children.Xerox and or fax pertinent documentation.Respond to telephone calls and take appropriate action.Participate in managing the offices so that it is free of hazards, clean and organized.Participate in staff meetings.Assist with special projects when required and perform other duties as assigned.Requirements:
LPN CertificationLPN LicenseUpdated First Aid/CPR certifications required2-3 years working with PediatricsCompletion of mandated course: Infection Control, Abuse/Incident Reporting
Salary Range: $37.12-$39.20
