JOB DESCRIPTION:
SUMMARY OF POSITION
Under the direction of the Program Manager, and the Medication-Assisted Treatment (MAT) team, the Licensed Practical Nurse (LPN) participates in treatment plan development and provides evaluation and treatment services to patients and their families suffering from opiate use disorder (OUD) and/or co-occurring physical and mental health conditions. Supports the team members in the provision of patient care to include direct patient care, providing healthcare information to patients/families, and following established plan of care. The LPN is responsible for ensuring coordination of care and checking vitals. They must have a positive attitude, follow directions well, communicate clearly and possess the ability to work independently when required.
Communicates clearly and appropriately; provides excellent customer service. Adheres to applicable policies and procedures. Regular attendance and punctuality is required.
Functions as staff LPN utilizing the nursing process with performing comprehensive psychiatric /mental health assessments. Evaluates mental status and refers clients to other treatment providers when necessary. Assumes primary responsibility and accountability for ones nursing practices as defined in the Indiana Code of Nursing Practice, as it pertains to a licensed practical nurse and functions under the supervision of an RN or physician.
ESSENTIAL DUTIES & RESPONSIBILITIES
Conducts nursing evaluations and assists in the admission process for new clients.Obtains vital signs for each patient prior to the prescriber visit.Obtains urine sample collection, documents and collaborates as neededMaintains client charting in accordance with professional standards.Provides client education regarding diagnosis, medication and health promotion.Participates in all assigned staff meetings and staff development programs.Performs venipuncture and other laboratory monitoring functions, as requested.KNOWLEDGE, SKILLS & ABILITIES
Must have the ability to work cooperatively with all staff and physicians and function as a valuable member of the treatment team. Effectively communicate via written, verbal, in person and virtual methods.Maintains a working knowledge of mental illness and the disease concept of chemical dependency, Mental Health and Chemical Dependency treatment, Alcoholics Anonymous philosophy, and group dynamics.Strong understanding of behavioral health diseases as well as alcohol and chemical dependency.Proficient in Microsoft Office productsCoordination, ability to organize, and plan are critical for this roleQUALIFICATIONS
Education Level
Graduation from an accredited nursing program is required.
Years of Experience
Minimum of six months to one year experience in psychiatric or equivalent mental health related nursing experience is preferred.
Certification/Licensure
Licensed Practical Nurse (LPN).

Hourly Compensation Range for TN: $23.10/hr. - $26.55/hr.
