The LPN supports DDI's residential programs and serves as a medical liaison for IDT team. The nurse updates the written plan for each person and maintains records for each individuals as well as appropriate in the planning and implementation of plan with DDI personnel. NY State Licensed Practical Nurse required.
Salary Range: $23.15-$27.78/hour
Responsibilities: Participates in IDT conferences, as required. Develops written plan for each person with RN Supervisor. Modifies nursing plan as necessary. Maintains medical records for each individual. Participates as appropriate in planning and implementation of training of DDI personnel. Accompanies individuals to medical appointments as assigned. Ensure proper medical follow-up through on call system. Ensures all medications are administered and stored as necessary. Attends monthly Nursing Meetings. Completes appropriate incident reporting paperwork as required. Ensures compliance with OPWDD regulatory standards. Liaison with family members regarding medical issues. Preparation of monthly Medication Administration Records. 24-hour on call for medical emergencies with rotating weekend coverage required. Qualifications: N.Y. State Licensed Practical Nurse License required N.Y.
