act as a liaison between patients, health care providers, and health insurers to ensure patients receive excellent health careprovide direct and indirect patient care services requiring technical and manual skills, within the scope of practice, that meet the psychosocial, physical and general aspects of caremaintain patient medical records and keep track of health outcomescommunicate the needs of the patient and familyconsider the patient's unique care needs and advocate for personalized treatment optionsdevelop and manage long-term care plans for patients with chronic or serious conditionseducate patients and their families on relevant health-related matters, and offering guidance for navigating complex medical decisionsperforms under the direction and supervision of a Registered Nursehave exceptional communication skills, organizational skills, and analytical and problem-solving skills QualificationsEducation and Work Experience
current and valid state license as a Licensed Practical Nurse (LPN/LVN)current BLS (AHA) certificate upon hire and maintain currentminimum one year experience in an applicable setting, preferredIV certification upon hire
