Licensed Practical Nurse (LPN) for Hospice Facility Per Diem

BrightStar Care of Westfield and Summit is currently hiring per diem Licensed Practical Nurses (LPN) to staff a local hospice facility in a relaxed, low-stress setting in Scotch Plains and/or Elizabeth. The main duties include passing medication. The working environment is very low stress and you get to choice when you want to work. 

The following per diem shifts are available on a week by week basis and includes a 30 minute unpaid break:

7:00am to 3:30pm

3:00pm to 11:30pm

11:00pm to 7:30am

Payrate:  $30/hr

Responsibilities

Assess patient's clinical status and environment, identifying needs for treatment and personal care addressing pain and discomfort

Ensure patient care needs are met as identified in patient assessments

Provide education, counseling, interventions as necessary to provide comfort care, maximizing quality of life for patient and family

Skilled nursing care as prescribed by physician which may include monitoring & tracking vitals, IV placement, comprehensive personal care, phlebotomy, medication and wound care

Responsible for ordering/reordering medication

Authorize, coordinate, supervise care of subordinate care providers per Plan of Care, and in accordance with state and federal regulations

Reports changes in condition; recommends orders to Shift Supervisor

Provides bereavement support to the family as appropriate

May pronounce at times of patient expirations, as needed

Qualifications and Requirements

Current unrestricted LPN license in state of NJ

Preferred: Bachelor’s degree in Nursing. Must have graduated from a National League of Nursing accredited program

Minimum 1 year of clinical experience under your LPN license

Clean background and criminal record

Understands regulatory compliance with state and federal regulations

Adhere to HIPAA and maintain client confidentiality

Current CPR, BLS or ACLS

Understands philosophy, principles of death/dying

Understand hospice and palliative care philosophies

Self-directed with the ability to work with minimal supervision

Possesses excellent observation, good nursing judgement and communication skills

Possesses and maintains good physical stamina and mental health

Flexible and cooperative in fulfilling role obligations
