Summary:
At Vighter, we take pride in partnering with the Department of Veterans Affairs (VA) to deliver compassionate healthcare services to our nation’s veterans. We are seeking dedicated Licensed Practical Nurses (LPN or LVN) to join our team and provide exceptional care at the New Orleans VA Medical Center in New Orleans, LA.
The LPN will utilize their broad-based clinical knowledge to effectively screen, implement health promotion and prevention interventions, manage acute and chronic illness states, assist patients/residents in the attainment of optimal levels of functioning rehabilitation, evaluate patient/resident response to actual or potential health problems, and provide supportive measures for the dying patient/resident and their significant others. Nursing care is performed in accordance with the Louisiana Practical Nurses Practice Act and within the individual nurse’s scope of license. The LPN will work under the guidance of a Registered Nurse to deliver comprehensive health services to patient/residents.

Principle Accountabilities:
+ Evaluate health care needs of the patient/resident and provide to RN to assist with developing a plan of care to meet interdisciplinary goals.
+ Notify RN of any significant changes in the patient’s/resident’s health condition.
+ Carry out the plan of care by performing assigned tasks.
+ Collect and document pertinent data into the electronic patient/resident medical record.
+ Complete nursing documentation within required time frames.
+ Identify and act on predictable needs of the patient/resident.
+ Prioritize and complete tasks within the LPN’s assigned shift.
+ Report incidents of verbal or physical abuse immediately to the Nurse Manager of the unit they are assigned; or designated off-shift nursing leadership.
+ Administer PO, IM, and subcutaneous medications including providing patient education.
+ Administer tube feedings and set up trays as needed.
+ Assist with cares including catheters, condoms, colostomies
+ Assist with mobility including transfers, transfer devices, and ambulation.
+ Educate patients/residents and/or families on medication administration.
+ Measure vital signs, including Pain the 5th Vital Sign, pulse oximetry, etc.
+ Measure intake and output for urine and drains.
+ Participate in admission and discharge of patient.
+ Perform wound care including dressing changes and obtaining wound cultures as necessary
+ Provide comfort measures including repositioning, bathing, activities of daily living, range of motion, and hygiene (shaving, brushing teeth, dressing).
+ Assist with routine urinary and bowel elimination including catheters, colostomies, and digital stimulation.
+ Respond and assist in an emergency situation as instructed.
+ Other duties as assigned.
Knowledge and Skills:
+ Must have exceptional customer service skills.
+ Employees shall be technically proficient in the skills necessary to fulfill the job requirements, including ability to speak, understand, read and write English fluently.
Education:
+ Certified as a Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN).
Qualifications:
+ The provider shall possess the required qualification/elements to be privileged and/or credentialed in accordance with the VetPro, background checks, fingerprinting, badging, annual fit testing, and pre-employment and annual drug testing.
+ The contractor’s credentials/qualifications are subject to review by VAMC Staff. These qualifications at a minimum will be based on Joint Commission (JC), or equivalent, quality standards for training, education, and competency.
+ Must have and maintain one of the following certifications: Basic Life Support for health care providers through the American Heart Association®, CPR for the professional rescuer through the American Red Cross or the American Safety and Health Institute.
+ Minimum of 2 (two) years of experience as a registered nurse in med surg or one of the specialties listed above .
+ Strong communication and interpersonal skills.
+ Commitment to patient-centered care and evidence-based medicine.
+ Ability to work independently while being a collaborative team player.
Duty Hours / On-call Requirements
+ Generally speaking, VAMCs are open 24/7/365. This position requires schedule availability in all shifts and rotational on-call duties and may necessitate return to the facility to provide care.
+ This position has availabilities to work day, evening and night shifts. Availability during weekends and holidays is a possibility. Advanced notice will be provided for scheduling purposes.
