Licensed Practical Nurse (LPN) -- Palmer Center Overview Requisition Number: 785 Date Posted: 1/28/2025 Location: Palmer Center Address: 1113 Flatbush Road City: Kingston Category: Clinical/Nursing Description The LPN at Palmer Center, under the supervision of a Registered Nurse, will coordinate and oversee the health care needs of an established caseload. Prior experience with the geriatric population or in a nursing home environment is helpful at Palmer Center. Responsibilities for this nursing position include, but are not limited to the following: + Administer medications and treatments to the individuals served according to the established Office for Persons With Developmental Disabilities (OPWDD) Approved Medication Administration Personnel (AMAP) Certification regulatory guidelines and agency policies. + Review the healthcare status of each individual served no less than quarterly. + Participate in the development, review, and update of an individual’s plan of care. + Provide for the development, with a physician, of medical care plan of treatment for an individual when the physician determines that such plan is required. + Implement appropriate and preventative healthcare measure included educating staff and individuals served in health and hygiene, infection control, detecting signs and symptoms of illness and basic first aid. + Provide specialized training to staff and individuals served as indicated by special needs. + Schedule medical appointments for the individuals served when indicated. Accompany or arrange for accompaniment of the individual to the appointment through the Program Manager. + Assure that all medical records are current, valid, and appropriately filed. + Report and take administrative direction from the RN, Sr. RN, and/or or Director of Nursing. + Communicate effectively with all staff to insure consistency, efficiency and follow through of healthcare needs of individuals served and program / agency goals. + Provide for and maintain the confidentiality of individuals served, their records, and their families. + Assures proper medical treatment is obtained in the event of injury, accident, or illness. + Assure appropriate follow up of protocol and OPWDD / agency procedures. HOURS: 40/daytime hours; may vary based on needs of the program/rotating weekends STARTING SALARY: $ 31.50 to $ 34.65 per hour Minimum Requirements + NYS Licensed Practical Nurse + Ability to function autonomously under the direction of an RN and/or Senior RN + Minimum one year of clinical experience as an LPN + Computer skills + OPWDD experience preferred
