A. PATIENT CARE
Collects data from patient, family or significant others and records on Initial Data Base form. Twenty-four hour nursing assessment and provides data to RN for MDS inclusion.
Contributes to the formulation and updating of plan of care and implements plan of care under the direction of the Registered Nurse.
Gives direct nursing care to assigned patients (e.g., medications, treatments, dressing changes, resident care, TPR) according to established standards.
Recognizes changes in patient condition and responds appropriately, notifying charge nurse as needed.
Documents pertinent information regarding patient's responses and nursing interventions in patient's medical record.
B. PATIENT CARE RELATED DUTIES
Applies principles of infection control and safety in the performance of patient care.
Communicates effectively with charge nurse and other member of the health care team regarding patient's needs and/or changes in condition.
Responds appropriately in crisis situation; able to prioritize.
Reports significant unit problems to the R.N. (charge nurse?)
Demonstrates understanding and complies with all regulatory agencies; ability to assume charge in the absence of an R.N. (charge nurse?)
Patient care responsibilities are delivered with a knowledge of patient growth and development and are appropriate to the ages of the patients served and complies with Bill of Rights.
Participates in patient teaching and nursing research activities (CQI).

Maintains established departmental policies and procedures, objectives, quality assurance program, safety, environmental, and infection control standards. Assesses gaps in policies and procedures, and create necessary policies and procedures to fulfill these gaps.

Orients patient and family to new environment; develops rapport with patients family and/or significant others. Orients LPNs, Nurses Aides (Certified Nurse Aides) and orderlies as required. Effectively utilizes the hospital computer system as it pertains to job responsibilities and in accordance with hospital policy. Participates on unit committees and in CQI activities as assigned. (stated above as well) Participates in patient care conferences and in staff development activities as required. Assists other personnel as needed. Performs other related duties as required. Graduation from an accredited school of practical nursing or equivalent education as described by the state education law. Current NJ license Licensed Practical Nurse license. Current American Heart Association Basic Life Support certification required. IV certification required Successful completion of Progressive Care Orientation Program required. Clinical nursing skills appropriate for an LPN. Good oral and written communication skills. Good interpersonal skills. Speaks, reads and writes English to the extent required by the position. All patient care areas; contact with patients, visitors, staff. Frequent prolonged standing/walking. Lifts, positions, pushes, and/or transfers patients.Protective clothing worn: face shield, gloves, impermeable gown, particulate respirator.
