Details:

The part-time LPN will provide medical care and services for women with a history of chemical dependence, including health history, physical examination, minor office procedures, treatment, referral, counseling, and follow-up care and services at Greenhope Services for women and children. 

The hourly rate for this role is $24.00/hr - $39.00/hr. 

RESPONSIBILITIES:

Under the supervision of a physician act as the health coordinator for outpatient and residential programs to provide services to women and children.Review referred potential clients' medical histories submitted by the intake department.Assist with referring clients to appropriate healthcare provider specialists.Assist with managing client prescriptions with various pharmacies, focusing on ChemRx.Coordinates with the client's primary care physicians to inform decisions regarding the management and treatment of problems identified and provides care as prescribed by the physician.Manage client care: follow up plan and monitor progress, determine when referral to another provider is necessary and make arrangements for further care, determine when emergency room visit is necessary and when illness is resolved.Work with interdisciplinary team members regarding modification of treatment as necessary according to health concerns.Provide oversight and monitoring of the wellness center to promote organized and efficient medication tracking, refills, and disposal of medication, as needed.Document all medications in the electronic health record system, AWARDS.May participate in staff meetings or professional development activities as needed.

 

EDUCATION AND REQUIREMENTS:  

Licensed Practical Nurse (LPN) or Registered Nurse from an accredited Nursing school with appropriate educational degrees.State of New York license and certification3+ years practical experience in health care facilityKnowledge of substance/alcohol dependence/abuse populationSpecialty areas may include but not limited to Family Health, Women's Health, Holistic Care, etc.BLS/CPR certification is required
