A professional care provider who assumes responsibility and accountability for a group of patients, providing care through the continued utilization of the nursing process in collaboration with other health team members. The LPN will be responsible for meeting the standards required by JCAHO, GMC and other regulating agencies for managing all assigned personnel, supplies and equipment on the unit. The LPN will process information on the Center's Health Information System as required in performing job responsibilities.
Duties/Responsibilities: Observes the organizations' attendance policy. Complies with departmental policies for reporting to work on time and for complying with designated meal/break period time frames. Attends and participates in department meetings. Identify self in a pleasant and positive manner. Assists patients, family members or other clients with concern and empathy. Demonstrates effective communication, recognizing diversity among age groups, cultures, and educational levels. Works consistently in a positive and cooperative manner with other employees in and outside the departmental unit. Supports final decisions even when such decisions may not entirely reflect one's own opinion. Values and incorporates the contributions of people from diverse backgrounds; demonstrates respect for the opinions and ideas of others. Is flexible regarding scheduling, shifts and department changes. Demonstrates commitment to excellence by looking for ways to improve and promote quality. Collects data in an ongoing and systematic manner, focusing on the biophysical, psychosocial, self-care, educational, discharge planning, and environmental needs of the patient. Implementation: Initiates plan of care based on established priorities, essential needs and available resources. Evaluation: reviews on an ongoing basis the effectiveness of intervention and reports finding to appropriate care provider. Demonstrates competence in required technical skills. Identifies learning needs in collaboration with health care team members and reinforces appropriate measures to meet patient needs. Accountable for overall medical record completeness and accuracy are documents in a timely, accurate, and concise manner reflecting proper use of terminology and abbreviations. Functions with an awareness and appreciation of safety/infection control policies as identified with the institution by using universal precautions and proper body mechanics.
Required: Graduate from an accredited school of Licensed Practical Nurses. Knowledge of current trends and techniques in nursing is appropriate to the practice setting. Participates in educational conferences, completes educational inservices and updates and maintains professional knowledge and skills related to areas of practice. Currently licensed by the State of North Carolina as a Licensed Practical Nurse. Up to date on CPR and CEU's for practice profession.
