Merakey is hiring an LPN to work in conjunction with our Adult Behavioral Health Case Management team in Delaware County.

 

Earn $31.20 per hour with benefits, M - F 9-5.

 

Within key functions in the Merakey Health Home model, the nurse navigator serves as a member of the Adult Targeted Case Management team. In this role, the nurse navigator provides care management, care coordination and health advocacy for consumers to ensure that they receive the behavioral healthcare and physical healthcare they need. The specialized case management services of the nurse navigator are provided to all individuals served by an Adult TCM team whether they participate in the on-site primary care services or receive those services from a PCP in the community.
