CVP is seeking Licensed Clinical Social Workers to join our team in providing our Nation's Veterans with high-quality care at the Southern Arizona VA Health Care System (SAVAHCS) in Tucson, AZ. We have openings for both In-Patient and Out-Patient Social Workers.
Location: Southern Arizona VA Health Care System (SAVAHCS), 3601 S. 6th Ave. Tucson, AZ 85723
*Out-Patient locations will vary
Work Schedule: Full-time, 40 hours per week. Specific shifts to be determined.
Contract Period: Base year plus four (4) one-year options, starting January 2025
Responsibilities Perform Biopsychosocial AssessmentsAssess patients and plan appropriately to assist with social determinants of healthConduct Suicide Risk, Mental Health, and Crisis Assessment and interventionDocument safety plans and other plans of care in the patient's medical recordProvide information and assist with completion of Advanced DirectivesAssess and complete process for Petition of Inpatient Psychiatric CareCommunicate pertinent patient data to providers and interdisciplinary teamReassess the patient's condition and revise plans of care as neededProvide resources and assistance with transportationSchedule and conduct family meetings, interdisciplinary team meetings, and goals of care conversationsProvide resources for veterans determined to be homelessAssess and address any findings of abuse or neglect, consulting Adult Protective Services as appropriateAssess and assist with discharge planning, including referrals to various care facilities and home health servicesIdentify veterans who need case management services post-dischargeAssess patient/family knowledge level, implement plan of instructions, and record demonstrated level of understandingAble to initiate, prioritize, and reorganize appropriate measures in emergency situationsCan initiate life-saving measures in the absence of PhysiciansAssures the content and correctness of a prepared report and signs the document to validate its contentReview all information in the electronic medical records pertinent for social work servicesMaintain professional decorum and appearance while consistently applying VA's customer service standardsRespects patient’s rights, including but not limited to visual and auditory privacy Qualifications Master of Science in Social Work (MSW) from a nationally accredited schoolCurrent, full, and unrestricted license to practice as a Social Worker in any State, Territory, or Commonwealth of the United States, or the District of ColumbiaMinimum of two years' current experience in Social WorkAbility to obtain and maintain VA credentials and privileges (U.S. Citizenship Required)Strong communication and interpersonal skillsProficiency in electronic medical record systemsAbility to work effectively in a multidisciplinary team environmentPreferred Qualifications:
Prior experience working with Veterans or in a VA settingFamiliarity with VA policies and proceduresExperience in either in-patient or out-patient settings, as applicable to the positionTo Apply: Qualified candidates should submit their resume, copy of license, and any relevant certifications.
