As a Licensed Vocational Nurse (LVN) at Tiburcio Vasquez Health Center, you will be responsible for providing professional standard outpatient nursing services. You will work under the daily supervision of Practice Management and in consultation with the Associate Director of Clinical Services. Your role will involve providing physical and psycho-social support to patients, their friends, and families, and acting as a patient advocate.
This is a full-time position, working 40 hours per week, typically Monday through Friday with periodic Saturday hours.
Compensation: $31.49 - $34.41 depending on experience

Responsibilities:
Provide nursing services, including but not limited to medication administration, immunizations, referrals to other providers and community health resources, and health instruction to patients or following examinations. Administer medication and treatment, ensuring documentation reflects care provided. Perform encounter forms and complete electronic health record entries (EPIC) for appropriate nurse-visits, such as health status checks, early urine pregnancy tests, and medication administration. Update patient charts, including reconciliation of medication list. Obtain medical reports from lab resulting agencies and outside healthcare providers, facilitating communication to healthcare providers. Obtain patient history for unscheduled patients and perform assessment data collection and review health history information for patients. Assist healthcare providers with in-clinic examination and procedures as appropriate, in coordination with Medical Assistant (MA) staff. Coordinate with Practice Management to ensure adequate medical supplies for the clinic. Act as a patient advocate, assess the learning needs of patients, and develop and implement patient education as appropriate. Attend general and medical staff meetings and participate in quality improvement projects. Participate in the development of policies and protocols for patient care. Perform all duties and services in full compliance with Tiburcio Vasquez Health Center's Service Excellence Standards.
