Licensed Vocational Nurse LVN / LPN Denton, TX (http://maps.google.com/maps?q=2214+Emery+Street,+Suite+410+Denton+TX+USA+76201) Job Type Part-time Description Trio Home Health (Jet Health, Inc)is looking for a PRN / Per DiemLicensed Practical Nurses (LPN / LVN)to join our dedicated team to provide community-based care for our patients currently receiving home health services in theNorth Denton and or Sanger, TX area. The primary function of the LPN / LVN is to provide nursing care, under the supervision of a registered nurse, for patients of all ages in their place of residence. The LPN / LVN will coordinate care with the interdisciplinary group, patient/family, and referring agency to ensure the best patient outcomes. Requirements + Graduate of an accredited School of Vocational/Practical Nursing. + Current State license as a Licensed Vocational/Practical Nurse, or a multi-state license issued by a state member of the National Nurse Licensure Compact (NLC) if practicing in a state member of the NLC. Must have a current driver's license. + Two years’ experience as a Licensed Vocational/Practical Nurse in a clinical care setting. Home health experience highly preferred. + HCHB experience highly preferred + Skills: Proficiency in nursing skills as defined by generally accepted standards of practice. Good interpersonal skills. + Reliable transportation and valid, current auto liability insurance. + Provide nursing care to patients of all ages in their homes under the supervision of a registered nurse. + Coordinate care with the interdisciplinary group, patient/family, and referring agency. + Assist in the development and implementation of patient care plans. + Monitor patient health and report any changes to the supervising RN. + Administer medications and treatments as prescribed. + Educate patients and their families on health care needs, conditions, and options. + Maintain accurate and timely documentation of patient care and progress. + Work in patients' homes in various conditions. + Ability to work a flexible schedule and travel locally. + Ability to lift up to 50 lbs and move patients. + Requires working under some stressful conditions to meet deadlines and patient needs. + Ability to make quick decisions and acquire necessary resources. + Must meet patient/family individualized psychosocial needs. + Requires hand-eye coordination and manual dexterity.
